摘要
目的探讨血清癌胚抗原(CEA)、铁蛋白(Fer)和糖基抗原199(CA199)在非小细胞肺癌诊断中的临床应用价值。方法分析62例初诊非小细胞肺癌患者的病例资料,采用德国罗氏Cobase411化学发光免疫分析仪联合罗氏CEA,Fer及CA199检测试剂盒检测样品血清中的相应指标,同时分析62例健康人群体检资料。结果非小细胞肺癌患者血清CEA、Fer和CA199含量显著高于健康人群(t=18.95、11.34、10.53,P<0.05)。非小细胞肺癌转移患者血清CEA、Fer和CA199含量显著高于非转移患者(t=11.75、9.26、6.83,P<0.05)。非小细胞肺癌腺癌患者血清CEA含量显著高于鳞癌患者(t=9.35,P<0.05)。受试者工作特征曲线分析表明,血清CEA、Fer和CA199鉴别诊断非小细胞肺癌的曲线下面积分别为0.784、0.721和0.68。结论血清CEA、Fer和CA199在鉴别诊断非小细胞肺癌中,CEA的临床诊断价值较大。
Objective To investigate the clinical value of serum carcinoembryonic antigen( CEA),ferritin( Fer) and carbohydrate antigen 199( CA199) in the diagnosis of non-small cell lung cancer. Methods The medical records of aforementioned markers from 62 cases of newly diagnosed non-small cell lung cancer patients and 62 cases of healthy population serum were analyzed by Roche Cobase411 chemiluminescence immunoanalyzer and relevant kit from Germany. Results The serum concentrations of CEA,Fer and CA199 in non small cell lung cancer patients were significantly higher than that of healthy population( t = 18. 95,11. 34,10. 53,P〈0. 05),in metastatic patients markedly higher than that of non metastatic patients( t = 11. 75,9. 26,6. 83,P〈0. 05). Moreover,serum concentration of CEA in adenocarcinoma patients was significantly higher than that of squamous cell carcinoma patients( t = 9. 35,P〈0. 05). The results of receiver operating characteristic curve( ROC) showed that the area under the curve of serum CEA,Fer and CA199 were 0. 784,0. 721 and 0. 68,respectively. Conclusion Compared with Fer and CA199,CEA can be particularly valuable to differential diagnosis between non-small cell lung cancer patient and healthy population.
