摘要
目的:研究可溶性协同刺激分子B7同系物4(soluble B7 homologue 4,s B7-H4)在食管癌前病变患者血清的表达水平,探讨s B7-H4诊断食管癌前病变的临床意义。方法:通过苏木素-伊红(hematoxylin-eosin,HE)染色方法,评价食管病理变化。通过酶联免疫吸附法(enzyme-linked immunosorbent assay,ELISA)检测血清s B7-H4和鳞状细胞癌相关抗原2(squamous cell carcinoma antigen-2,SCCA-2)的浓度。结果:食管癌前病变分为低级别上皮内瘤变(low-grade intraepithelial neoplasia,LGIN)和高级别上皮内瘤变(high-grade intraepithelial neoplasia,HGIN)。HGIN患者血清SCCA-2浓度显著高于LGIN患者和健康受试者血清SCCA-2浓度(P<0.01,P<0.01)。LGIN患者血清s B7-H4浓度显著高于健康受试者血清s B7-H4浓度(P<0.05),并且显著低于HGIN患者血清s B7-H4浓度(P<0.01)。s B7-H4在血清的表达与食管组织病理级别呈显著正相关(P<0.0001),而与患者的年龄和性别无关(P=0.7777,P=0.6068)。结论:食管癌前病变进程中血清s B7-H4浓度逐渐提高,s B7-H4对于判断食管癌前病变进展程度,是一个有价值的血清标志物。
Objective: To study the expression of soluble costimulatory molecule B7 homologue 4(s B7-H4) in sera of patients,in order to investigate the clinical significance of s B7-H4 in the diagnosis of esophageal precancerous condition. Methods: Esophageal pathological changes were evaluated by hematoxylin-eosin(HE) staining. s B7-H4 and squamous cell carcinoma antigen-2(SCCA-2) concentrations were detected by enzyme-linked immunosorbent assay(ELISA). Results: The precancerous lesions of the esophagus were divided into low grade intraepithelial neoplasia(LGIN) and high grade intraepithelial neoplasia(HGIN). The serum SCCA-2 concentration in HGIN patients was significantly higher than that in LGIN patients and healthy subjects(SCCA-2,P 0.01,P 0.01). The serum s B7-H4 concentration in LGIN patients was significantly higher than that in healthy subjects(s B7-H4)(P 0.05),and significantly lower than that in HGIN patients(P0.01). The expression of s B7-H4 in serum was positively correlated with the histopathological grade of esophagus(P 0.0001),but not related to age and sex(P = 0.7777,P = 0.6068). Conclusion: The concentration of serum s B7-H4 increased progressively in the course of precancerous lesions of the esophagus. s B7-H4 is a valuable serum marker for judging the progression of precancerous lesions.
