PCI术前负荷剂量瑞舒伐他汀对老年非ST段抬高急性冠状动脉综合征患者近期预后的影响

Effect of Pre-PCI Loading Dose of Rosuvastatin on Short-term Prognosis of Elderly Patients with Non ST Segment Elevation Acute Coronary Syndrome

目的分析PCI术前负荷剂量瑞舒伐他汀对老年非ST段抬高急性冠状动脉综合征(NSTEACS)患者近期预后的影响。方法随机选取我院2015年6月至2017年6月84例NSTEACS患者作为研究对象,分成研究组和对照组各42例,其中研究组于PCI术前12 h服用20 mg瑞舒伐他汀及PCI术前2 h加服用20 mg瑞舒伐他汀。并于术前、术后24 h、术后1个月抽取两组NSTEACS患者外周静脉血,检测并比较其Lox-1、hs-CRP、c TnⅠ、CK-MB的情况。随访1个月,观察比较两组NSTEACS患者心脏不良事件的发生率。结果术后24 h两组NSTEACS患者的Lox-1、hs-CRP、c TnⅠ、CK-MB显著高于术前(P<0.05),但研究组Lox-1、hs-CRP、c TnⅠ、CK-MB显著低于对照组(P<0.05),术后1个月研究组Lox-1、hs-CRP仍明显较对照组低(P<0.05);随访1个月,研究组NSTEACS患者心脏不良事件的发生率显著低于对照组(P<0.05)。结论 PCI术前负荷剂量瑞舒伐他汀除了本身具有的调脂作用外,还有利于降低老年NSTEACS患者PCI术后Lox-1水平,缓解患者术后的心肌损伤和炎症反应,较少术后心脏不良事件的发生率。

Objective To analyze the effect of pre-PCI loading dose of rosuvastatin on the short-term prognosis of elderly patients with non ST segment elevation acute coronary syndrome（ NSTEACS）. Methods Eighty-four cases of NSTEACS patients treated in our hospital between June 2015 and June 2017 were randomly selected as subjects and divided into the study group and control group（ 42 cases each）. The study group took 20 mg rosuvastatin 12 h and 2 h before PCI. The peripheral venous blood of the two groups was extracted before operation,24 h and 1 month after operation. The levels of Lox-1,hs-CRP,c Tn I and CKMB were compared. These patients were followed up for 1 month. The incidence of adverse cardiac events in the two groups was observed and compared. Results The levels of Lox-1,hs-CRP,c Tn,and CK-MB in the two groups were significantly higher24 h after operation（ P〈0. 05）,but they were significantly lower in the study group than in the control group（ P〈0. 05）. The case was the same one month after operation. The incidence of adverse events was significantly lower in the study group than in the control group（ P〈0. 05）. Conclusion A loading dose of rosuvastatin before PCI can help reduce the level of Lox-1 after PCI operation in elderly patients with NSTEACS,alleviate postoperative myocardial injury and inflammation,with fewer postoperative adverse cardiac events.
