黄芪多糖注射液对哮喘患者支气管灌洗液或痰液炎症细胞计数和相关因子水平的影响

Effect of Astragalus Polysaccharide Injection on Inflammatory Cell Count and Levels of Related Factors in Bronchial Lavage Fluid or Sputum of Patients with Asthma

目的研究黄芪多糖注射液对哮喘患者支气管肺泡灌洗液(BALF)或痰液炎症细胞计数和相关因子水平的影响。方法采取随机数字表法将我院收治的196例住院支气管哮喘患者分为两组,各98例。对照组予以常规治疗,观察组基于常规治疗使用黄芪多糖注射液,比较两组治疗前后BALF白细胞介素6(IL-6)、白介素8(IL-8)、肿瘤坏死因子α(TNF-α)与痰液白介素13(IL-13)、白介素17(IL-17)、免疫相关指标、肺功能、不良反应及疗效。结果观察组疗效总有效率为89.80%,对照组为81.63%,两组比较无显著差异(P>0.05);治疗2周后,观察组BALF IL-6、IL-8、TNF-α与痰液IL-13、IL-17细胞计数均明显低于对照组(P<0.05),且CD3^+、CD4^+、CD4^+/CD8^+值、FEV1、VC、PEF明显高于对照组(P<0.05);观察组不良反应总发生率为2.04%,明显低于对照组的10.20%(P<0.05)。结论对支气管哮喘患者在常规治疗基础上予以黄芪多糖注射液,可有效减少BALF或痰液炎症细胞计数,提高免疫因子水平,增强肺功能,疗效确切,且不良反应少,具有积极应用价值。

Objective To study the effect of Astragalus polysaccharide injection on inflammatory cell count and levels of related factors in the bronchoalveolar lavage fluid（ BALF） or sputum of patients with asthma. Methods One hundred and ninetysix cases of patients with bronchial asthma treated in our hospital were evenly divided into two groups using the random number table method. The control group was given conventional treatment,while the observation group was additionally treated with Astragalus polysaccharide injection. The levels of interleukin 6（ IL-6）,interleukin 8（ IL-8） and tumor necrosis factor α（ TNF-α） in BALF,interleukin 13（ IL-13） and interleukin 17（ IL-17） in sputum,immune related indexes,pulmonary function,adverse reactions and curative effect were compared between the two groups. Results There was no significant difference in the total effective rate of treatment between the two groups（ 89. 80% vs 81. 63%）（ P〈0. 05）. After two weeks of treatment,the levels of IL-6 IL-8 and TNF-α in BALF,IL-13 and IL-17 in sputum of the observation group were significantly lower than those in the control group（ P〈0. 05）,while levels of CD3~＋, CD4~＋,CD4~＋/CD8~＋,FEV1,VC and PEF were significantly higher（ P〈0. 05）. The total incidence of adverse reactions in the observation group was significantly lower than that in the control group（ 2. 04% vs 10. 20%）（ P〈0. 05）. Conclusion The application of Astragalus polysaccharide injection combined with conventional treatment in the treatment of patients with bronchial asthma can effectively reduce the inflammatory cell count in BALF or sputum,increase the levels of immune factors and improve lung function,with fewer adverse reactions.
