奥卡西平与卡马西平对老年三叉神经痛患者血浆P物质水平及睡眠质量的影响比较

Effects of oxcarbazepine and carbamazepine on plasma substance P level and sleep quality in elderly patients with primary trigeminal neuralgia

目的:观察奥卡西平(OXC)与卡马西平(CBZ)对老年三叉神经痛(PTN)患者血浆P物质(SP)水平及睡眠质量的影响比较。方法:按随机数表法将142例老年PTN患者分为OXC组(研究组)和CBZ组(对照组)各71例。于治疗前(T1)、治疗2个月后(T2)评估2组患者疼痛程度[简化Mc Gill疼痛问卷-2(SF-MPQ-2)]和睡眠质量[理查兹-坎贝尔睡眠量表(RCSQ)],检测两组患者血浆P物质(SP)、β-内啡肽(β-EP)水平,观察不良反应情况。结果:T2时,两组SF-MPQ-2和RCSQ评分、血浆SP水平均较T1时降低,且研究组低于对照组(P均<0.05);血浆β-EP水平则较T1时升高,且研究组高于对照组(P均<0.05)。研究组不良反应发生率低于对照组(P<0.05)。结论:OXC对老年PTN患者疼痛、睡眠改善情况优于CBZ,且不良反应发生率较CBZ低,更具安全性。

Objective： To observe the effects of oxcarbazepine（ OXC） and carbamazepine（ CBZ） on plasma substance P（ SP） level and sleep quality in elderly patients with primary trigeminal neuralgia（ PTN）. Methods： 142 cases of elderly PTN patients were divided into OXC group（ study group） and CBZ group（ control group） with 71 cases in each group according to the random number table method. The pain degree [short-form Mc Gill Pain Questionnaire-2（ SF-MPQ-2） ]and sleep quality [Richards-Campbell Sleep Questionnaire（ RCSQ） ] were assessed in the two groups before treatment（ T1） and after 2 months of treatment（ T2）,and the levels of plasma substance P（ SP） and β-endorphin（ β-EP） were measured in the two groups,and adverse reactions were observed. Results： At T2,the scores of SF-MPQ-2 and RCSQ and the plasma SP level in the two groups were lower than those at T1,and the indexes in study group were lower than those in control group（ all P〈0. 05）. The plasma β-EP level was higher than that at T1,and the level in study group was higher than that in control group（ all P〈0. 05）. The incidence rate of adverse reactions in study group was lower than that in control group（ P〈0. 05）. Conclusions： OXC is superior to CBZ in the improvement of pain and sleep in elderly PTN patients,and the incidence rate of adverse reactions is lower than that of CBZ,and OXC has higher safety.
