上饶市儿童乙肝疫苗接种效果评价及乙肝血清学调查验证

Evaluation of Hepatitis B Vaccination Effect in Children of Shangrao City and Investigation of hepatitis B serological investigation

目的:了解上饶市儿童乙肝疫苗接种后乙肝感染、发病及免疫水平,为进一步调整乙肝疫苗免疫策略提供依据。方法:收集整理2006~2015年上饶市乙肝发病、乙肝疫苗接种资料。抽取600份健康人群血液标本检测乙肝表面抗原(HBs Ag)、表面抗体(抗-HBs),对数据进行统计分析。结果:全市儿童乙肝疫苗接种率逐年上升。0~14岁儿童乙肝发病率逐年下降,但≥15岁的乙肝发病率呈明显上升趋势,且主要集中在20~35周岁。抽样调查结果显示:HBs Ag携带率为16.17%,15岁以下儿童乙肝携带率为10.83%,≥15岁年龄组阳性率(23.69%),两组差异有统计学意义(χ~2=17.789,P<0.05)。全市人群抗-HBs阳性率为77%,且0~14岁组阳性率(84.33%)高于≥15岁年龄组阳性率(66.7%),两组差异有统计学意义(χ~2=25.663,P<0.05)。结论:接种乙肝疫苗是预防与降低乙肝感染的重要途径,在加强儿童乙肝疫苗接种的同时需关注成人乙肝免疫接种。

Objective： To investigate the infection,morbidity and immune status of hepatitis B virus in children of Shangrao after hepatitis B vaccination,and to provide the basis for further adjustment of hepatitis B vaccine immunization strategy. Methods： The data of hepatitis B virus and hepatitis B vaccination in Shangrao City were collected from 2006 to 2015. HBs Ag Hepatitis B surface antigen（ HBs Ag）,surface antibody（ anti-HBs） were detected by blood samples from600 healthy people,The results were analyzed by χ^2 test and other statistical methods. Results： Statistical analysis of the results of the city＇s hepatitis B vaccine vaccination rate increased year by year. The incidence of hepatitis B in children aged 0 - 14 years decreased year by year,but the incidence of hepatitis B was significantly higher at 15 years of age and older,and mainly in the 20 - 35 age group. Sampling survey showed that HBs Ag carrying rate was 16. 17%. Children under the age of 15 hepatitis B carrying rate was 10. 83%,≥ 15 years old positive rate（ 23. 69%）,the difference was statistically significant（ χ^2= 17. 789,P〈0. 05）,The positive rate of HBs Ag in grouP〈0 - 14 was lower than that in ≥15 years old group. The positive rate of anti-HBs was 77%,and the positive rate（ 84. 33%） in the 0 - 14 year group was higher than that in the age group ≥ 15 years old（ 66. 7%）,and the difference was statistically（ χ^2= 25. 663,P〈0. 05）. Conclusion： Hepatitis B vaccination is an important way to prevent and reduce hepatitis B infection. At the same time,attention should be paid to adult hepatitis B immunization with hepatitis B vaccine.
