牙周基础治疗对龈沟液中自噬水平的影响

Effect of periodontal treatment on autophagy level in gingival crevicular fluid

目的通过比较牙周治疗前后牙周临床指标和龈沟液中自噬水平的变化,评估自噬水平对牙周炎治疗效果的反应性和相关性。方法对21例成人牙周炎患者进行全口龈上洁治和龈下刮治,在每位患者口内预先选取一颗牙周袋深度≥5 mm的患牙,分别在治疗前和治疗后两周时检测该患牙龈沟液中自噬因子的浓度,并记录牙周临床指标进行统计分析。结果经过牙周基础治疗,牙龈指数(GI)、牙周探诊深度(PD)和临床附着丧失(CAL)均有明显降低,龈沟液中微管相关蛋白1轻链3(LC3)和Beclin-1的表达水平亦有下调,差异均有统计学意义(P<0.05)。治疗前,自噬水平与GI、PD、CAL均表现为正相关(P<0.05);治疗后自噬水平仅与GI表现为正相关(P<0.05)。ΔGI和ΔLC3、ΔBeclin-1则均为正相关(P<0.001)。结论牙周基础治疗可以有效地控制成人牙周炎患者龈沟液中自噬的表达水平,自噬表达水平能够一定程度判断牙周炎的治疗效果。

Objective To investigate the periodontal clinical outcome and autophagy level in gingival crevicular fluid（ GCF）before and after periodontal treatment.Methods Twenty-one adult patients with periodontitis were recruited to receive periodontal scaling and root planning.The indexes of periodontal clinical outcome and GCF autophagy level were measured before and after 2 weeks of treatment.Results After periodontal scaling and root planning,gingival index（ GI）,probing depth（ PD） and clinical attachment loss（ CAL） were significantly decreased.The expression levels of microtubule-associated protein 1 light chain 3（ LC3） and beclin-1 in GCF were decreased（ P〈0. 05）.Before periodontal treatment,autophagy level had positive correlation with all three periodontal outcome indexes,but after treatment autophagy level was only positively correlated to GI（ P〈0. 05）.Regarding the decrease level,ΔGI was positively correlated to ΔLC3 and Δbeclin-1（ P〈0. 001）. Conclusion For adult periodontitis patients,periodontal treatment can effectively control the level of autophagy in GCF which can be used to assess clinical effect of the treatment.
