摘要
目的探讨动态监测血氧灌注指数(PI)在足月新生儿休克的诊断和治疗中的价值。方法选取2016年1月至2017年4月我院新生儿科收治的86例足月休克新生儿,按新生儿休克评分分为轻度、中度、重度休克,选取同期收治的80例高危新生儿为对照组。休克组新生儿在诊断为休克及治疗后1d、2d检测PI值,对照组新生儿在出生后2 h、1d、2d检测PI值,比较两组PI值的变化。结果研究组新生儿确诊休克时、治疗后1d的PI值均低于对照组(P<0.05)。研究组新生儿治疗后2d两组比较差异无统计学意义(P>0.05);研究组中重度、中度、轻度三组休克新生儿在确诊休克时、治疗后1d的PI值差异有统计学意义(P<0.05),三组在治疗后2d的PI值比较差异无统计学意义(P>0.05)。结论动态检测PI值在足月新生儿休克的诊断以及治疗中具有良好的临床疗效,值得临床推广应用。
Objective To investigate the value of dynamic monitoring of oxygen perfusion index( PI) in the diagnosis and treatment of neonatal shock in full-term newborns.Methods A total of 86 newborns with neonatal shock admitted to our hospital from January 2016 to April 2017 were selected as subjects.The patients were divided into mild,moderate and severe shock groups according to neonatal shock score.At the same time,80 high risk newborn were selected as control group.The PI values were examined at the shock and after 1 day and 2 days of treatment.The PI values of the neonates in the control group were measured at 2 h,1 d and 2 d after birth.The PI values were compared between the two groups. Results The PI value at the shock and after 1 day of treatment in the shock group was lower than that in the control group( P〈0. 05).However,there was no significant difference between the two groups after 2 days treatment( P〈0. 05).The differences of the PI values at the diagnosis of shock and after 1 day of treatment among the groups of severe,moderate and mild shock was statistically significant( P〈0. 05) but not after 2 days of treatment( P〈0. 05). Conclusion Dynamic detection of oxygen perfusion index in the diagnosis and treatment of neonatal shock in full-term newborns has a good clinical effect.It is worthy of clinical application.
