上皮性卵巢恶性肿瘤的预后分析

Prognostic analysis of epithelial ovarian cancer

目的比较不同组织学亚型的上皮性卵巢恶性肿瘤的临床病理特征,并对其预后情况进行分析。方法选取2008年1月至2015年12月在我院接受手术治疗的上皮性卵巢恶性肿瘤患者328例,收集患者临床病理信息并随访至2017年7月。随后依据2014版WHO卵巢上皮性肿瘤分类标准对所有肿瘤进行分类,对比各类型卵巢癌的临床病理特点和预后情况。结果上皮性卵巢恶性肿瘤患者的5年无进展生存率约为30%;5年总存活率约为45%。不同亚型的卵巢癌在FIGO分期(P<0.0001)、术前CA125水平(P<0.0001)以及生存期(PFS:P=0.0004;OS:P=0.028)比较,差异有统计学意义。FIGO分期晚和术前CA125水平较高的患者的无进展生存期和总生存期都更短(P<0.0001)。结论不同组织亚型的卵巢癌具有不同的临床病理特征和预后。此外,FIGO分期晚和术前CA125水平较高的患者预后不良。

Objective To compare the clinicopathological features and prognosis of epithelial ovarian cancers with different histological subtypes.Methods Clinicopathological data and follow-up information（ till July 2017） of 328 ovarian cancer patients who undertook surgery between January 2008 and December 2015 in our hospital was collected.Histological subtypes of each tumor were defined according to criteria of the World Health Organization（ 2014 edition）.Clinicopathological features and prognosis of each histological subtype were compared.Results Five-year progression free survival rate of these patients was approximately 30%.Five-year overall survival rate was about 45%.There was statistically significant difference in FIGO stages（ P〈0. 0001）,preoperative CA125 levels（ P〈0. 0001） and survival time（ PFS： P = 0. 0004; OS： P = 0. 028） among patients with different histological subtypes. Patients with advanced FIGO stage and high preoperative CA125 levels had shorter PFS and OS（ P〈0. 0001）.Conclusion Different histological subtypes of ovarian cancers have different clinicopathological characteristics and prognosis.Patients with advanced FIGO stage and high preoperative CA125 levels have poor prognosis.
