脉冲射频治疗带状疱疹后神经痛临床效果的系统评价 被引量:25

CLINICAL EFFECT OF PULSED RADIOFREQUENCY TREATMENT ON POSTHERPETIC NEURALGIA: A SYSTEMATIC REVIEW

摘要 目的:评价脉冲射频治疗带状疱疹后神经痛的镇痛效果及有效性,为临床治疗提供参考。方法:数据库检索Pub Med、The Cochrane Library、CNKI、VIP、Wan Fang Data、中国生物医学文献数据库,时限从1997年1月1日至2016年10月1日。根据纳入排除标准,收集脉冲射频治疗带状疱疹后神经痛的随机对照试验,依据Cochrane Handbook 5.1.0偏倚风险评估工具评价纳入文献质量,文献数据用Revman 5.3软件进行Meta分析。结果:最终21项随机对照试验纳入研究,共1463名受试者。Meta分析结果表明:脉冲射频组比对照组有更好的镇痛效果,同时有更高的有效率、显效率和优良率,统计结果有显著性意义。结论:与传统方法相比,脉冲射频治疗6个月内临床效果更好,且术后并发症少,是理想的治疗方法,但远期效果需进一步探讨。 Objective: To evaluate the analgesic effect and effectiveness of pulsed radiofrequency (PRF) on postherpetic neuralgia (PHN) and to provide the basis and the reference for clinical treatment. Method: A sys- tematic and comprehensive database search was performed in PubMed, The Cochrane Library, China National Knowledge Infrastructure (CNKI), China Science and Technology Journal Database (VIP), Chinese Biomedical Literature Database, and Wanfang database for literatures published since January 1, 1997 to October 1, 2016. According to inclusion and exclusion criteria, randomized controlled trials presenting PRF for PHN treatment were collected. The risk of bias tool described in the Cochrane Handbook version 5.1.0 was used to assess the quality of each trial. Meta-analysis was performed using RevMan 5.3 software. Results: A total of twenty-one randomized controlled trials involving 1 463 patients met inclusion criteria. The results of recta-analysis showed that, compared with the control group, PRF had a better effect on pain score, effective rate, marked effective rate, excellent and good rate. Conclusion: Compared with the traditional method, PRF shows an obvious analgesic effect and a lower incidence of adverse reaction for the treatment of PHN in 6 months. It is an ideal method for the treatment of PHN, but long-term effects need to be further discussed.
