白细胞介素-2对狼疮肾炎血尿酸水平变化的影响及意义

Changes and significance of serum uric acid in lupus nephritis treated with interleukin 2

目的:探讨白细胞介素-2对于狼疮肾炎患者血尿酸改变的临床效果及意义。方法:选取首次确诊为系统性红斑狼疮患者及复发未使用过环磷酰胺或霉酚酸酯的40例患者作为研究对象,给予患者白介素联合传统药物治疗,统计分析患者治疗前、后血清尿酸、24 h尿蛋白定量及血肌酐情况。结果:经过治疗,患者血清尿酸由(431.38±58.44)μmol/L降至(274.73±18.84)μmol/L,治疗前后比较,差异具有统计学意义(均P<0.05)。24 h尿蛋白定量由(2.34±0.67)g/d降至(0.58±0.03)g/d,治疗前后比较,差异具有统计学意义(均P<0.05);血肌酐由(221±31.49)μmol/L降至(121±19.08)μmol/L,治疗前后比较,差异具有统计学意义(P<0.05)。结论:狼疮肾炎患者的血尿酸水平可以及时反映其肾脏损伤程度,结合本研究患者血尿酸指标变化情况,白介素2联合传统治疗可显著恢复患者肾功能,改善血尿酸指标,有利于患者预后。

Objective To investigate the clinical effect and significance of interleukin 2 on serum uric acid in patients with lupus nephritis. Method 40 patients with systemic lupus erythematosus and 40 cases of relapsed unused cyclophosphamide or mycophenolate mofetil were selected as the subjects. The patients were treated with interleukin combined with traditional medicine. The serum uric acid,24 h urine protein quantitative and serum creatinine were statistically analyzed before and after treatment. Results After treatment,the blood uric acid level of the patient began to decrease obviously at first weeks. The serum uric acid was reduced from（ 431. 38 ± 58. 44） μmol/L to（ 274. 73 ± 18. 84） μmol/L. The difference was statistically significant before and after treatment（ P〈0. 05）; 24 h urine protein was reduced from（ 2. 34 ± 0. 67） g/d to（ 0. 58 ± 0. 03） g/d,the difference was statistically significant before and after treatment（ P〈0. 05）; renal function decreased from（ 221 ± 31. 49）μmol/L to（ 121 ± 19. 08） μmol/L,and the difference was statistically significant before and after treatment（ P〈0. 05）. Conclusion The level of blood uric acid in patients with lupus nephritis can respond to the degree of renal injury in time. Combined with the change of the index of uric acid in this study,the combination of interleukin 2 combined with traditional treatment can significantly restore the renal function of the patients,improve the index of uric acid and be beneficial to the prognosis of the patients.
