重型颅脑损伤患者运用超声引导下改良塞丁格技术行PICC置管的效果观察 被引量:7

Observation of the effect of PICC catheterization in patients with severe craniocerebral injury treated with modified Seldinger technique guided by ultrasound

摘要 目的:探讨超声引导下改良塞丁格技术行PICC置管对重型颅脑损伤患者的效果。方法:选取88例重型颅脑损伤患者为研究对象,以随机数字表法分组,试验组与对照组各44例,对照组采用盲穿法改良塞丁格技术行PICC置管,试验组在超声引导下以改良塞丁格技术行PICC置管,对两组患者置管情况进行观察。结果:试验组一次穿刺成功率为97.73%,一次置管成功率为97.73%,对照组分别为81.82%和79.55%,两组差异有统计学意义(P<0.05);试验组PICC置管后静脉炎、穿刺点出血、血栓等并发症发生率为4.55%,与对照组27.27%相比,明显较低,两组差异有统计学意义(P<0.05)。结论:重型颅脑损伤患者应用超声引导下改良塞丁格技术行PICC置管具有可行性,可准确定位靶血管,提高一次穿刺成功率与一次置管成功率,减少局部组织创伤,也能降低并发症发生风险,值得临床推广应用。 Objective To study the ultrasound guided by improved plug dingle technology line of PICC catheter for patients with severe craniocerebral injury. Method 88 cases of severe craniocerebral injury patients were selected as the research object,grouping by the method of random Numbers table,experimental group( n = 44) and the control group( n = 44),and control group with traditional approach line of PICC catheter,the experimental group in under the guidance of ultrasound with improved dingle technology line of PICC catheter,catheter on two groups of patients were observed. Results The experimental group a puncture success rate of 97. 73%,a catheter success rate was 97. 73%,the control group were 81. 82% and 79. 55%,respectively,the difference between the two groups was statistically significant( P〈0. 05); Experimental phlebitis after PICC catheter,the puncture point,the incidence of complications such as hemorrhage,thrombosis,at 4. 55%,compared with the control group27. 27%,significantly lower,the difference between the two groups was statistically significant( P〈0. 05). Conclusion Patients with severe head injury under ultrasound improved plug dingle technology line of PICC catheter with feasibility,can accurate positioning target blood vessels,improve a success rate of puncture success rate with a catheter,reduce local tissue trauma,can also reduce the risk of complications,worthy of popularization and application.
