指状式冰敷模型在吻合器痔上黏膜环切术中及术后的应用效果

Application of Finger-Shaped Ice Compress Model in the Procedure for Prolapse and Hemorrhoids

目的探讨指状式冰敷模型在吻合器痔上黏膜环切术(procedure for prolapse and hemorrhoids,PPH)术中及术后的应用效果。方法便利抽样选择2017年1-6月山东省聊城市人民医院肛肠科收治的混合痔患者107例为研究对象。按入院先后分为观察组(n=52)与对照组(n=55)。对照组给予常规术后护理,观察组采用自制指状冰敷模型于术中、术后早期应用于PPH手术患者。比较两组患者术后切口疼痛、水肿、出血及尿潴留发生情况。结果观察组术后6h、术后首次排尿疼痛程度均明显低于对照组;术后8h,观察组轻度肛门水肿占38.46%、中度水肿44.23%,对照组轻度肛门水肿占20.0%、中度水肿45.45%;术后24h,观察组术后出血3例(5.77%),对照组术后出血15例(27.27%);术后12h,观察组发生尿潴留2例(3.85%),对照组发生尿潴留13例(26.0%)。上述指标组间比较,差异均有统计学意义(均P<0.05)。结论指状式冰敷模型可以减轻PPH患者术后疼痛、水肿及出血,有效预防尿潴留。

Objective To investigate the application effect of finger-shaped ice compress model in the procedure of prolapse and hemorrhoids（PPH）.Methods 107 patients with mixed hemorrhoid underwent PPH were randomly divided into observation group（n=52）and control group（n=55）.The control group was given routine postoperative care,and the observation group was treated with self-made finger ice compress model during and after the operation.Postoperative pain,edema,bleeding and urinary retention were compared between the two groups.Results Six hours after operation,the first urination pain in the observation group was significantly lower than those in the control group.Eight hours after operation,the observation group had 38.46% of mild anal edema and 44.23% of moderate edema;the control group had 20%of mild anal edema and 45.45%of moderate edema.24 hours after surgery,there were 3 cases（5.77%）had postoperative bleeding in the observation group,and 15 cases（27.27%）in the control group.After 12 postoperative hours,the observation group had 2 cases of urinary retention（3.85%）,and the control group had13 cases of urinary retention（26%）.All the indexes above were statistically significant（all P〈0.05）.Conclusions Finger-shaped ice compress model can reduce pain,edema and bleeding after PPH,and effectively prevent urinary retention.
