长春市养老机构护士资源配置现状及影响因素

State Quo of Nurse Resource Allocation and Influencing Factors in Pension Institutions in Changchun City

目的:分析长春市养老机构护士资源的配置状况和影响因素,为地方医养结合政策的进一步实施和完善提供依据。方法:以长春市283个已取得设立许可证且正在运营的养老机构为研究对象,对养老机构的基本情况、护士配置数量等进行问卷调查;采用描述统计、卡方检验和logistic回归分析等分析养老机构护士资源配置水平、分布状况和重要影响因素。结果:无护士配备的养老机构占总数的79.0%,配备1名护士的养老机构占12.0%,配备2名及以上护士的养老机构占9.0%。护士资源配置在不同规模、性质和区域机构间存在显著差异;养老机构护士资源配置主要受机构规模、分布区域、是否是失能人员医疗照护保险定点机构和是否收住失智老人等因素的影响。结论:长春市养老机构护士配置水平较低,应出台相关政策提高养老机构配置护士资源的意愿和能力,推动养老机构资源和医疗资源互融互通,促进我国医养结合的养老事业发展。

Objective： To explore the nurse resource allocation and influencing factors in pension institutions in Changchun City,so as to provide the reliable basis for improvement of integrated health and social care policy. Methods： A representative sample of 283 pension institutions in Changchun City was investigated. A validated quantitative pension institutions questionnaire was used to assess the basic situation of the pension institutions,the number of nurses. Descriptive statistics,chi square test and logistic regression analysis were used to analyze the resource allocation level,distribution of nurse resources and important influencing factors in pension institutions. Results： 79.0% of pension institutions had no nurse,12.0% of pension institutions had 1 nurse,and 9.0% of pension institutions had more than 2 nurses. There were differences in distribution of nurse resources in the bed size,distribution area and institutional ownership. Influencing factors of the nurse resource allocation were bed size,distribution area,the agency with disability medical care insurance and dementia elderly. Conclusion： There are fewer resources of nurses in pension institutions in Changchun City. The policy should be taken to promote nurse staffing levels. Pension institutions are encouraged to promote integrating with medical resources,so as to develop the integrated health and social care.
