腹腔镜胃癌根治术治疗老年胃癌的临床研究

Clinical Study on Laparoscopic Radical Gastrectomy for Elderly Patients with Gastric Cancer

目的探讨腹腔镜胃癌根治术治疗老年胃癌的临床疗效。方法选取我院2014年1月至2017年6月接受腹腔镜胃癌根治术的34例老年患者(腹腔镜组),另选取2012年1月至2014年1月接受开腹胃癌根治术的30例老年患者(开腹手术组)。比较两组患者的围术期指标、术后疼痛和并发症情况。结果腹腔镜组的术中出血量和住院时间均优于开腹手术组(P<0.05)。腹腔镜组术后4 d、7 d的VAS评分均低于开腹手术组(P<0.05)。腹腔镜组的术后并发症发生率为8.82%,低于开腹手术组的30.00%(P<0.05)。结论腹腔镜胃癌根治术能够明显促进患者术后恢复,降低疼痛程度,减少并发症的发生,值得临床推广。

Objective To explore the clinical curative effect of laparoscopic radical gastrectomy for elderly patients with gastric cancer. Methods 34 cases of elderly patients in our hospital from January 2014 to June 2017 receiving laparoscopic radical gastrectomy were selected as the laparoscopic group, and another 30 cases of elderly patients from January 2012 to January 2014 receiving laparotomy were selected as the laparotomy group. The perioperative indicators, postoperative pain and complications were compared between the two groups. Results The intraoperative blood loss and hospitalization time of the laparoscopic group were lower than those of the laparotomy group（P〈0.05）. The VAS scores at 4 d and 7 d after operation of the laparoscopic group were lower than those of the laparotomy group（P〈0.05）. The incidence of postoperative complications of the laparoscopic group was 8.82%, lower than 30.00% of the laparotomy group（P〈0.05）. Conclusions Laparoscopic radical gastrectomy can significantly promote the postoperative recovery, reduce the pain degree and complications, which is worthy of clinical promotion.
