颅内动脉瘤夹闭术后脑脊液乳酸浓度与预后的相关性

Correlation between CSFL Concentration and the Prognosis after Intracranial Aneurysm Clipping

摘要 目的研究探讨CSFL浓度与动脉瘤夹闭术后患者预后的相关性。方法检测颅内动脉瘤夹闭术后CSFL浓度以及同时采取的血乳酸浓度,根据CSFL浓度正常组(A组),异常组(B、C、D),统计不同组别之间患者预后以及与并发症发生率之间的差异。结果 B组与A组GOS评分比较无明显差异,C组、D组与A组GOS评分比较明显降低(P<0.05),且C、D两组较A、B两组之间的脑积水、脑梗死的发生率明显升高(P<0.05)。结论 1.CSFL浓度越高,并发症发生率越高,颅内动脉瘤患者预后越差;2.CSFL>3.0mmol/L时,预后较差,CSFL浓度可作为动脉瘤术后早期评价预后的一项参考指标。 Objective To explore the correlation between the concentration of cerebrospinal fluid lactate( CSFL)and the prognosis of the patients after intracranial aneurysm clipping.Methods CSFL and blood lactate of 68 cases after intracranial aneurysm clipping were collected and the concentrations were detected; according to the concentration of CSFL,the cases were divided into 4 groups: group A( normal group),group B( abnormal group),group C( abnormal group) and group D( abnormal group); the prognosis and complication occurrence in the 4 groups were statistically studied and the differences were compared.Results The Glasgow outcome score( GOS) in group B was of no obvious difference from that in group A while the scores in group C and group D were much lower than that in group A( P〈0. 05); the occurrences of hydrocephalus and cerebral infarction in group C and group D were higher than those in group A and group B( P〈0. 05).Conclusions The higher the concentration of CSFL is,the higher the occurrence of complications is and the poorer the prognosis is in cases after intracranial aneurysm clipping; CSFL3. 0 mmol/L leads to poorer prognosis,the concentration of CSFL after intracranial aneurysm clipping may taken as a reference in the early evaluation of prognosis.
