阶梯式颅内减压技术在急性重型颅脑损伤治疗中的临床效果分析

Analysis of Clinical Effect of Staircase Intracranial Decompression Technique in the Acute Severe Cerebral Injury

目的分析探讨对急性重型颅脑损伤患者使用阶梯式颅内减压技术的临床治疗效果。方法将2016年1月—2017年12月收治的96例急性重型颅脑损伤患者作为研究对象,随机分48例为接受传统减压术治疗的对照组患者,以及余下48例为接受阶梯式颅内减压术治疗的观察组,分析比较两组患者的手术治疗以及并发症发病等情况。结果观察组患者的手术时间、术中出血量及并发症发病率分别为(64.23±16.34)min、(113.29±24.39)mL、25.00%,和接受传统颅内减压术的对照组的(79.92±21.49)min、(192.91±52.11)mL、47.92%相比均显著,差异有统计学意义(P<0.05);观察组病患治疗的良好率为62.50%(30/48),和对照组29.17%(14/48)的良好率相比显著较高,差异有统计学意义(χ~2=9.604;P=0.000<0.05)。结论为急性重型颅脑损伤患者使用阶梯式颅内减压技术治疗可有效减少手术时间,减少术中出血量及并发症发生率,且治疗良好率较高,临床治疗效果较高,具有临床广泛推广价值。

Objective To analyze and study the effect of staircase intracranial decompression technique in the acute severe cerebral injury. Methods 96 cases of patients with acute severe cerebral injury admitted and treated in our hospital from January 2016 to December 2017 were selected and randomly divided into two groups with 48 cases in each,the control group and the observation group were respectively treated with traditional decompression technique and staircase intracranial decompression technique, and the operation treatment and occurrence of complications of the two groups were analyzed and compared. Results The operation time, intraoperative bleeding amount and incidence rate of complications in the observation group were obviously better than those in the control group, [（64.23 ±16.34）min,（113.29±24.39）mL, 25.00% vs（79.92±21.49）min,（192.91±52.11）mL, 47.92%],,the difference was statistically significant（P〈0.05）, and the treatment good rate in the observation group was obviously higher than that in the control group,[62.50%（30/48） vs 29.17%（14/48）],the difference was statistically significant（χ2=9.604; P =0.000 0.05）. Conclusion The application of staircase intracranial decompression technique in the acute severe cerebral injury can effectively reduce the operation time, reduce the intraoperative bleeding amount and incidence rate of complications with higher treatment good rate and clinical treatment effect, and it is worth clinical wide promotion value.
