经直肠彩色多普勒超声检查在前列腺癌诊断中的临床价值分析

Analysis of Clinical Value of Transrectal Color Doppler Ultrasonography in Diagnosis of Prostatic Cancer

目的探析经直肠彩色多普勒超声检查在前列腺癌诊断中的应用价值。方法选择该院2014年6月—2017年10月收治的41例前列腺癌患者为观察组,另选同期45例前列腺增生症患者为对照组,均进行经直肠彩超检查,比较两组病灶边界、形态及大小以及前列腺内动脉血流情况差异。结果 (1)二维超声显示,观察组病灶形态不对称(82.93%)、边界模糊(75.61%)、低回声结节(68.29%)、回声不均(80.49%)、簇状钙化(87.80%)几率高于对照组(P<0.05);(2)CDFI显示,两组病灶血流分级比较差异有统计学意义(χ~2=44.865,P=0.000),观察组以Ⅱ~Ⅲ级血流信号为主,对照组则为0~Ⅰ级血流为主。此外,观察组的Vmax值、RI值均较对照组明显升高(P<0.05)。结论经直肠彩色多普勒超声可清晰显示肿块边界、形态大小、内部回声及血流分布情况,利于临床鉴别诊断前列腺癌,具有较高的临床应用价值。

Objective To study the application value of transrectal color Doppler ultrasonography in diagnosis of prostatic cancer. Methods 41 cases of patients with prostatic cancer admitted and treated in our hospital from June 2014 to October 2017 were selected as the observation group, while 45 cases of patients with hyperplasia of prostate at the same period were selected as the control group for transrectal color Doppler ultrasonography, and the lesion boundary,shape and size and internal arterial blood flow of the prostate were compared between the two groups. Results The twodimensional ultrasound showed that the occurrence probabilities of lesion asymmetry（82.93%）, blurred boundary（75.61%）, hypoechoic nodules（68.29%）, uneven echo（80.49%）, and clustered calcification（87.80%） were higher than those in the control group,（P〈0.05）, and the CDFI showed that the difference in the lesion blood flow classification between the two groups was obvious（χ2=44.865, P=0.000）, and the blood flow signal of level Ⅱ-Ⅲ was the main in the observation group and of level 0-Ⅰ in the control group, besides, the Vmax value and RI value obviously increased compared with those in the control group（P〈0.05）. Conclusion The transrectal color Doppler ultrasonography can clearly show the tumor boundary, shape and size, inner echo and blood flow distribution, which is conductive to the clinical differential diagnosis of prostatic cancer, and the clinical application value is higher.
