高通量血液透析对维持性血液透析患者甲状旁腺激素、Hcy和左心室结构的影响 被引量:5

摘要 目的探讨高通量血液透析对维持性血液透析患者甲状旁腺激素(i PTH)、同型半胱氨酸(Hcy)和左心室结构的影响。方法将2015年8月~2017年4月因尿毒症于本院就诊的90例患者纳入研究并随机分组。对照组45例用低通量血液透析,观察组45例采用高通量血液透析,6个月为1疗程。比较两组临床疗效差异。结果治疗后观察组肌酐、尿素氮等肾功能指标低于对照组,差异有统计学意义(P<0.05);治疗后观察组Hcy、左心室舒张末内径(LVDd)、甲状旁腺素低于对照组,LVEF更高(P<0.05);观察组总有效率93.33%(42/45),高于对照组的73.33%(33/45),差异具有统计学意义(P<0.05)。结论高通量血液透析治疗维持性血液透析患者效果显著,可有效保护肾功能,预防心室重构,调节甲状旁腺激素,值得推广。 Objective To investigate the effects of high-flux hemodialysis on i PTH, Hcy and left ventricular structure in maintenance hemodialysis patients. Methods From August 2015 to April 2017, 90 patients with uremia in our hospital were included in the study and randomly divided into groups. 45 cases of control group with low-pass hemodialysis, 45 cases of observation group using high-throughput hemodialysis, 6 months for a course of treatment. The difference of clinical efficacy between the two groups was compared. Results After treatment, creatinine, urea nitrogen and other renal function indexes in the observation group were lower than those in the control group(P〈0.05). Hcy, LVDd and parathyroid hormone in the observation group were lower than those in the control group LVEF was higher(P〈0.05). The total effective rate in observation group was93.33%(42/45), which was higher than that in control group(73.33%)(P〈0.05). Conclusion High-throughput hemodialysis is effective in patients with maintenance hemodialysis and can effectively protect renal function, prevent ventricular remodeling and regulate parathyroid hormone, which is worth promoting.
