恩度联合顺铂胸腔内注射治疗癌性胸水的疗效及安全性分析

Efficacy and safety of Endostar combined with cisplatin in the treatment of malignant pleural effusion by intrapleural injection

目的探讨恩度联合顺铂胸腔内注射治疗癌性胸水的效果及安全性观察。方法将2015年12月~2017年7月因癌性胸水于本院就诊的80例患者纳入研究并随机分组,对照组40例患者单纯顺铂(顺铂60 mg/m2+0.9%氯化钠注射液200 ml)胸腔内注射,观察组40例则联合恩度治疗,21 d为一个化疗周期,均行1个周期。比较两组临床疗效及不良反应情况。结果观察组有效率77.50%(31/40),高于对照组的55.00%(22/40),差异有统计学意义(P<0.05);治疗后观察组VEGF水平低于对照组(P<0.05);两组不良反应均主要表现为恶心呕吐、骨髓抑制、肝功能损伤、疲劳,发生率组间比较差异无统计学意义。结论恩度联合顺铂胸腔内注射治疗癌性胸水效果显著,可降低VEGF水平,安全可靠,值得推广。

Objective To evaluate the efficacy and safety of Endostar combined with cisplatin in the treatment of malignant pleural effusion.Methods 80 patients with cancerous hydrothorax in our hospital from December 2015 to July 2017 were randomly divided into two groups. 40 patients in the control group were injected with cisplatin（cisplatin 60 mg/m^2＋0.9% Sodium Chloride Injection 200 ml） in the thoracic cavity, 40 cases in the observation group and 21 days as a chemotherapy cycle, and all the 1 cycles were performed. The clinical effects and adverse reactions of the two groups were compared. Results The effective rate of the observation group was 77.50%（31/40）, which was higher than 55%（22/40） of the control group（22/40）, and the difference was significant（P〈0.05）. The level of VEGF in the observation group was lower than that of the control group（P〈0.05）. The two group of adverse reactions were mainly manifested as nausea and vomiting, bone marrow suppression, liver function injury and fatigue, and there was no significant difference between the groups. Conclusion Endostar combined with cisplatin is effective in the treatment of malignant pleural effusion, which can reduce the level of VEGF. It is safe and reliable, and is worthy of promotion.
