硬腰联合麻醉对老年髋关节置换术患者麻醉效果及血流动力学的影响

Influence of hard waist combined anesthesia on anesthetic effect and hemodynamics in elderly patients with hip joint replacement

目的探讨硬腰联合麻醉对老年髋关节置换术患者麻醉效果及对血流动力学的影响。方法选取我院386例老年髋关节置换术患者为研究对象,将其随机分为对照组和研究组,各193例,分别给予硬膜外麻醉及硬腰联合麻醉。比较两组患者的麻醉效果、不同时间点血流动力学指标变化及麻醉恢复情况。结果对照组麻醉后30min及术后5min的SBP、DBP、HR较研究组波动较大(P<0.05),两组SV及SpO_2在不同时间点比较无统计学差异(P>0.05);研究组患者麻醉药物剂量、麻醉起效时间及阻滞完全时间均显著优于对照组(P<0.05);麻醉优良率显著高于对照组(P<0.05);研究组患者麻醉苏醒时间、意识恢复时间及呼吸满意时间均显著短于对照组(P<0.05)。结论硬腰联合麻醉对血流动力学指标影响小,麻醉效果显著,值得应用推广。

Objective To explore influence of hard waist combined anesthesia on anesthetic effect and hemodynamics in elderly patients with hip joint replacement. Methods A total of 386 elderly patients with hip joint replacement in our hospital were selected as objects and randomly divided into control group and study group, with 193 cases in each group. The two groups were given epidural anesthesia and hard waist combined with anesthesia respectively. The anesthetic effect, change of hemodynamic parameters at different time points and anesthesia recovery were compared between the two groups. Results Thirty minutes after anesthesia and 5 minutes after surgery, the changes of SBP, DBP and HR of the control group were more than those in the study group（P 〈0.05）. There was no significant difference in SV and SpO2 between the two groups at different time points（P〉0.05）. The anesthetic drug dose, the onset time of anesthesia, and the complete time of blockade in the study group were significantly better than those in the control group（P〈0.05）. The good and better rate of anesthesia in the study group was significantly higher than that in the control group（P 〈0.05）. The anesthesia recovery time,consciousness recovery time, and respiratory satisfaction time in the study group were significantly shorter than those in the control group（P 〈0.05）. Conclusion Hard waist combined anesthesia has little effect on hemodynamic parameters, with significant anesthetic effect, it is worthy of application and promotion.
