腹腔镜胆囊切除术对胆结石的治疗效果

Effect of laparoscopic cholecystectomy on cholelithiasis

目的探讨腹腔镜胆囊切除术治疗胆结石的临床疗效。方法回顾性分析2015年1月至2017年5月130例行腹腔镜手术(观察组)和40例行开腹手术(对照组)的胆结石患者的临床资料。比较两组患者的治疗效果。结果两组患者的治疗总有效率比较,无差异明显(P>0.05)。观察组患者的手术耗时、下床时间、住院时间均显著短于对照组,差异具有统计学意义(P<0.05)。对照组患者均顺利完成手术,观察组中1例患者发现胆囊周围严重黏连,中转开腹顺利完成手术。观察组患者的并发症发生率为0.0%,显著低于对照组的5.0%,差异具有统计学意义(P<0.05)。结论腹腔镜胆囊切除术治疗胆结石患者的手术创伤小、术后恢复快。但存在胆囊和腹腔严重黏连等危险情况者,要及时中转开腹处理,以避免引发严重的术后并发症。

Objective To investigate the clinical effect of laparoscopic cholecystectomy on cholelithiasis. Methods The clinical data of 130 cases of patients with laparoscopic surgery（observation group） and 40 cases of patients with open abdominal surgery（control group） from January 2015 to May 2017 were retrospectively analyzed. The therapeutic effects of the two groups were compared. Results The total therapeutic efficiency of the two groups was not significantly different（P 〉0.05）. The time of operation, time of get ting out of bed and hospital stay in the observation group were significantly shorter than those of the control group, the differences were statistically significant（P〈0.05）. All the patients in the control group completed the operation successfully. One patient in the observation group was found severe adhesion around the gallbladder, and was conversion to laparotomy and completed the operation successfully. The incidence of complications in the observation group was 0.0%, which was significantly lower than 5.0% in the control group, the difference was statistically significant（P〈0.05）. Conclusion Laparoscopic cholecystectomy in patients with gallstones has minimal surgical trauma and rapid postoperative recovery. However, patients with dangerous conditions such as severe adhesion of gallbladder and abdominal should timely conversion to laparotomy, to avoid serious postoperative complications.
