SchatzkerⅤ、Ⅵ型胫骨平台骨折的手术方法及效果

Surgical methods and effects of Schatzker Ⅴ and Ⅵ tibial plateau fractures

目的探讨SchatzkerⅤ、Ⅵ型胫骨平台骨折的手术方法及效果。方法选择本科室2016年1月至2017年1月收治的80例SchatzkerⅤ、Ⅵ型胫骨平台骨折患者,按随机数表分为内固定组(n=40,双侧钢板内固定)与结合组(n=40,有限内固定结合外固定)。比较不同手术方案的近期及远期效果。结果两组患者术后1年的膝关节功能恢复优良率比较,无明显差异(P>0.05);结合组患者术中出血量少于内固定组,手术时间、骨折愈合时间短于内固定组(P<0.05)。结论 SchatzkerⅤ、Ⅵ型胫骨平台骨折实施有限内固定结合外固定治疗手术创伤小,术后恢复快,膝关节功能恢复良好。

Objective To explore the surgical methods and effects of the Schatzker Ⅴ and Ⅵ tibial plateau fractures.Methods Eighty patients with Schatzker Ⅴ and Ⅵ tibial plateau fractures treated in our department from January 2016 to January 2017 were selected and randomly divided into internal fixation group（n =40, bilateral internal fixation） and combination group（n=40, limited internal fixation combined with external fixation）. The short-term and long-term effects of different surgical methods were compared. Results There was no significant difference in excellent and good rates of the knee joint function between the two groups at 1 year after surgery（P〉0.05）. The intraoperative blood loss in the combined group was less than that in the internal fixation group, and the operation time and fracture healing time were shorter than those in the internal fixation group（P〈0.05）. Conclusion The limited internal fixation combined with external fixation in Schatzker Ⅴ and Ⅵ tibial plateau fractures has small surgical trauma, rapid postoperative recovery and good recovery of knee function.
