全自动血细胞分析仪联合血涂片细胞形态学检查在血液系统疾病筛查中的作用 被引量:11

摘要 目的探讨全自动血细胞分析仪联合血涂片细胞形态学检查在血液系统疾病筛查中的作用。方法选取于我院行血常规检查的患者586例,所有患者均接受血常规检查,先使用全自动血细胞分析仪检测,再使用血涂片镜检进行复检。对比两种方法的检测结果及经全自动血液分析仪检测为阳性患者细胞形态的诊断结果。结果 586份血常规标本中,需复检137例,复检率为23.38%(137/586)。全自动血液分析仪的灵敏度为84.04%、特异度为88.21%、误诊率为11.79%、漏诊率为15.96%、诊断符合率为87.54%。血细胞分析仪对原始细胞、单核细胞、幼稚细胞诊断符合率较低,假阳性率较高。结论全自动血细胞分析仪联合血涂片细胞形态学检查在血液系统疾病筛查可提高检测的准确性,为血液系统临床诊断的重要依据。 Objective To investigate the role of automatic blood cell analyzer combined with blood smear cell morphology in blood system disease screening. Methods A total of 586 patients were selected for routine blood examination in our hospital. All the patients received routine blood examination. The results were first tested using a fully automatic blood cell analyzer, and then the blood smear mirror was used for re-examination. The results of the two methods and the cell morphology of the patients who were positive by automatic blood analyzer were compared. Results Among 586 specimens,137 cases re-examined, the re-examination rate was 23.38%(137/586). The sensitivity of the automatic blood cell analyzer was 84.04%, the specificity was 88.21%, the misdiagnosis rate was 11.79%, and the missed diagnosis rate was 15.96%, the diagnostic coincidence rate was 87.54%. The automatic blood cell analyzer had low diagnostic coincidence rate for the primordial cells, monocytes and immature cells, and the false positive rate was high. Conclusion Automatic blood cell analyzer combined with blood smear cell morphology observation in blood system disease screening can improve the accuracy of detection, which is an important basis for clinical diagnosis of blood system disease.
