血小板输注无效的影响因素及交叉配型的应用效果

Influencing factors of platelet transfusion failure and the application effect of cross-matching

目的研究血小板输注无效的影响因素及交叉配型的效果。方法选择我院2016年11月至2017年11月收治的血小板输注患者80例为研究对象。通过问卷调查的方式收集所有患者的一般资料,采用Logistic回归分析引起输注无效的危险因素。结果80例患者中,输注无效患者30例。单因素分析发现,性别、输血次数、抗体类型、血液疾病是输注无效的影响因素(P<0.05);Logistic回归分析显示,女性、输血大于3次、抗-HLA抗体、合并白血病是输注无效的独立危险因素(P<0.05)。结论导致血小板输注无效的危险因素较多,但经交叉配型后完全配合的血小板输注,效果较为明显,患者的输血情况能够得到明显的改善,值得临床进一步研究。

Objective To study the influencing factors of platelet transfusion failure and the effect of cross-matching.Methods Eighty patients with platelet transfusion from November 2016 to November 2017 in our hospital were selected as the study objects. The general data of all patients were collected by questionnaire, and the risk factors for platelet transfusion failure were analyzed by Logistic regression analysis. Results Among 80 patients, there were 30 cases of ineffective platelet transfusion. The univariate analysis showed that gender, times of blood transfusions, antibody types and blood diseases were the influential factors of PTR（P 〈0.05）. The Logistic regression analysis showed that women, blood transfusion more than 3 times, anti-HLA antibody, complicated with leukemia were independent risk factors of PTR（P〈0.05）. Conclusion There are many risk factors that lead to PTR. The effect of platelet transfusion with complete combination after cross-matching is obvious, and the blood transfusion condition of patients can be improved obviously, which is worthy of further clinical study.
