欣普贝生引产胎膜早破68例临床分析

Dinoprostone induced premature rupture of membranes: clinical analysis of 68 cases

目的探讨欣普贝生用于足月头位引产的安全性及是否引起胎膜早破发生。方法回顾性分析2015年3月1日至2018年3月1日西安安琪儿妇产医院和成都安琪儿妇产医院足月单胎头位欣普贝生引产1 626例,其中发生胎膜早破68例设为研究组,同时选取足月单胎头位自发胎膜早破180例设为对照组。比较两组分娩方式及母婴结局。结果足月单胎头位欣普贝生引产总阴道分娩率为78.62%;胎膜早破发生率为4.18%。研究组阴道分娩率为61.76%,高于对照组的47.22%,差异显著(P<0.05);两组产时/术时出血量、新生儿Apgar评分及出生体重比较,差异均无统计学意义(P>0.05);产后胎盘、胎膜送病理检查中,两组绒毛膜羊膜炎发生率比较,差异无统计学意义(P>0.05)。结论欣普贝生用于足月头位可促进宫颈成熟及引产安全、可靠,极少引起胎膜早破的发生,阴道分娩率高,并可使缩宫素引产的成功率大大提高。

Objective To discuss the safety of dinoprostone inducing labor applied in head presentation in full-time p regnancy and whether premature rupture of membranes caused by it. Methods A retrospective analysis was made of 1626 cases of women at full term（single fetus and head presentation） of Xi’an Angela Women’s and Children’s Hospital and Chengdu Angel Women’s and Children’s Hospital from 1 March, 2015 to 1 March, 2018, including 68 cases of premature rupture of membrane s as the study group. At the same time, 180 cases of spontaneous prematured rupture membranes（full term, single fetus and head presentation） were selected as control group. The mode of delivery and outcomes of the mother and child in the two groups were compared. Results The total vaginal delivery rate of dinoprostone inducing labor applied in women（full term, single fetus and head presentation） was 78.62%, the incidence of premature rupture of membranes was 4.18%. The rate of vaginal delivery of the study group was 61.76%, which was higher than 47.22% of the control group（P〈0.05）. There were no significant differences in the amount of bleeding, Apgar score and birth weight of the two groups（P 〉0.05）. In the pathological examination of postpartum placenta and fetal membranes, the incidences of chorioamnionitis of the two groups had no significant difference（P 〉0.05）. Conclusion Dinoprostone inducing labor applied in head presentation in full-time pregnancy can promote cervical ripening, and it’s safe and reliable, it rarely causes premature rupture of membranes, with high rate of vaginal delivery, and it can greatly improve the success rate of oxytocin induced abortion.
