髋关节置换和内固定修复治疗老年股骨颈骨折疗效及安全性分析

Therapeutic Effect and Safety Analysis of Hip Replacement and Internal Fixation for Femoral Neck Fractures in the Elderly

目的探究髋关节置换和内固定手术在治疗老年股骨颈骨折中的疗效及安全性差异。方法抽取2016年1月—2018年3月140例来该院接受治疗的老年股骨颈骨折患者作为研究对象。内固定组采用空心加压螺钉置入内固定疗法,置换组采取全髋关节置换疗法,2组髋关节Harris评分对比,2组关节功能满意度调查。结果置换组优良率90.00%高与内固定组62.85%,差异有统计学意义(χ~2=6.51,P<0.05)。置换组并发症病例较少5.71%,内固定组28.57%,置患组显著低于内固定组(χ~2=5.93,P<0.05)。置换组总满意率92.86%较高,内固定组78.57%,差异有统计学意义(χ~2=4.80,P<0.05)。结论髋关节置换较内固定更有效稳定骨折股骨,预防进一步骨折,且临床具有良好安全性,是治疗老年股骨颈骨折更有效、安全的治疗方法。

Objective This paper tries to investigate the efficacy and safely of hip replacement and internal fixation in the treatment of femoral neck fractures in the elderly. Methods 140 elderly patients with femoral neck fractures treated in this hospital from January 2016 to March 2018 were selected as study subjects. In the internal fixation group, hollow compression screws were used for internal fixation, the replacement group was treated with total hip replacement, the Hart＇is hip scores of the two groups were compared, and the satisfaction of the joint function of the two groups was in- vestigated. Results The excellent and good rate of the replacement group was 90.00% and that of the internal fixation group was 62.85%, the difference was statistically significant （X2=6.51, P〈0.05）. The complications in the replacement group were 5.71% less, 28.57% in the internal fixation group, and significantly lower in the placement group than in the internal fixation group （X2=5.93,P〈0.05）. The total satisfaction rate was 92.86% in the replacement group and 78.57% in the internal fixation group, the difference was statistically significant （X2=4.80, P〈0.05）. Conclusion Hip replacement is more effective than internal fixation to stabilize the fractured femur, prevent further fractures, and has good clinical safety. It is a more effective and safe treatment for femoral neck fractures in the elderly.
