摘要
目的:探究梅毒所致心血管疾病的超声影像表现及其诊断价值。方法:回顾性分析在我院诊治的48例梅毒性心血管疾病患者(梅毒组)的超声影像学资料,并与48例健康人(健康对照组)进行比较,分析其主要表现:心脏瓣膜厚度和主动脉直径等。结果:48例梅毒性心血管疾病患者超声心动图主要表现为瓣膜改变、主动脉关闭不全、房室腔改变,以及心肌功能下降等;与健康对照组比较,梅毒组患者主动脉的左冠瓣[(20.5±5.2)mm比(25.9±3.2)mm]、右冠瓣[(21.2±4.7)mm比(25.3±3.9)mm]以及无冠瓣[(20.9±4.2)mm比(24.9±4.4)mm],二尖瓣的前叶[(28.3±5.1)mm比(35.1±5.8)mm],三尖瓣的前叶[(24.9±3.7)mm比(28.2±4.1)mm]和膈叶[(22.1±2.8)mm比(27.2±3.4)mm]的厚度均明显增厚(P均=0.001),主动脉根径[(27.2±3.1)mm比(31.3±5.1)mm]、升主动脉内径[(26.9±5.4)mm比(33.4±6.3)mm]显著增加(P均=0.001)。结论:超声检查对梅毒所致心血管疾病具有重要的诊断价值,心脏瓣膜厚度的增加,主动脉根径以及升主动脉内径的增加,是其超声图像的重要表现。
Objective:To explore ultrasound imaging performance of syphilis-induced cardiovascular diseases and its diagnostic value.Methods:Ultrasound imaging data of 48 patients with syphilitic cardiovascular disease(syphilis group),who were diagnosed and treated in our hospital,were retrospectively analyzed.They were compared with those of 48 healthy subjects(healthy control group),their main manifestations:heart valve thickness and aortic diameter etc.were analyzed.Results:Main echocardiographic manifestations of syphilis group were valvular changes,aortic regurgitation,atrioventricular cavity changes,and reduced myocardial function etc.;compared with healthy control group,there were significant rise in thickness of left coronary valve [(20.5±5.2)mm vs.(25.9±3.2)mm],right coronary valve[(21.2±4.7)mm vs.(25.3±3.9)mm],no coronary valve [(20.9±4.2)mm vs.(24.9±4.4)mm],mitral anterior lobe[(28.3±5.1)mm vs.(35.1±5.8)mm],tricuspid anterior lobe[(24.9±3.7)mm vs.(28.2±4.1)mm]and diaphragmatic lobe[(22.1±2.8)mm vs.(27.2±3.4)mm,P=0.001 all],aortic root diameter[(27.2±3.1)mm vs.(31.3±5.1)mm]and ascending artery diameter[(26.9±5.4)mm vs.(33.4±6.3)mm]in syphilis group,P=0.001 both.Conclusion:Ultrasound examination possesses important diagnostic value for syphilis-induced cardiovascular disease.Heart valve thickness,aortic root diameter,and ascending artery diameter increase are important manifestations of its ultrasound imaging.
