NT-proBNP、H_2S和HCgp-39在川崎病中的表达及意义 被引量:5

The expression and significance of NT-proBNP,H_2S and HC gp-39 in Kawasaki's disease

摘要 目的探讨血浆氨基末端脑钠肽前体(NT-proBNP)、内源性硫化氢(H_2S)以及人软骨糖蛋白39(HC gp-39)在川崎病中的表达及意义。方法前瞻性选取2016年2月至2017年11月治疗的川崎病患儿74例作为观察组,同时选取健康儿童60例作为对照组。检测两组NT-proBNP、H_2S和HC gp-39水平以及血小板参数[血小板计数(PLT)、血小板压积(PCT)、平均血小板体积(MPV)和血小板分布宽度(PDW)]、左室舒张末期内径(LVEDD)和左室射血分数(LVEF)。分析观察组患儿NT-proBNP、H_2S和HC gp-39水平与血小板参数、LVEDDI和LVEF的相关性。结果观察组急性期NT-proBNP和HC gp-39水平明显高于恢复期和对照组(P<0.05),而H_2S明显低于恢复期和对照组(P<0.05);观察组恢复期PLT和PCT明显高于急性期和对照组(P<0.05),而MPV和PDW明显低于急性期和对照组(P<0.05);观察组急性期、恢复期和对照组LVEDD比较差异无统计学意义(P>0.05);观察组急性期LVEF明显低于恢复期和对照组(P<0.05);NT-proBNP与LVEF呈负相关(r=-0.424,P<0.04);H_2S与PLT呈正相关(r=0.315,P<0.05),与MPV呈负相关(r=-0.401,P<0.05);HC gp-39与PLT呈正相关(r=0.310,P<0.05)。结论川崎病患儿NT-proBNP和HC gp-39水平升高,H_2S水平降低,其中NT-proBNP与患儿左室收缩功能有关,H_2S和HC gp-39与患儿血小板参数有关。 Objective To investigate the expression and significance of plasnm N - ten, final pro brain natriuretic peptide ( NT - proB- NP) , endogenous hydrogen sulphide (H2 S) and human cartilage glycoprotein 39 (HC gp -39 ) in Kawasaki's disease. Methods From Februm7 2016 to November 2017, 74 cases of children with Kawasaki disease in our hospital (obseccation group) , and 60 healthy children were selected as control group, the relationship of NT-proBNP, H2S and HC gp -39 level and platelet parameters [ platelet count (PLT) , plateletcrit (PCT) , mean platelet volume (MPV) and platelet distribution width (PDW) ], left ventricular end diastolic diameter (LVEDD) and left ventricular ejec- tion fraction (LVEF) were analyzed. Results The NT - proBNP and HC gp - 39 levels in acute stage of observation group were significantly higher than that in the recovery period and the control group ( P 〈 0.05 ) , and H2S was significandy lower than that in the recovery period and the control group ( P 〈 0.05). The PLT and PCT in recovel7 phase of the observation group were significantly higher than those in the acute phase and the control group ( P 〈 0.05 ) , while MPV and PDW were significantly lower than those in the acute phase and the control group ( P 〈 0.05 ). There was no significant difi^rence in LVEDD in the acute period, the recovery period of the observation group and the control group ( P 〉 0.05 ). In the acute phase of the observation group, the LVEF was significantly lower than that in the recovery period and the control group ( P 〈 0.05). NT - proBNP was negatively correlated with LVEF ( r - - 0. 424, P 〈 0.04 ). H2 S was positively correlated with PLT ( r - 0. 315, P 〈 0.05 ), and was negatively correlated with MPV ( r - - 0. 401, P 〈 0.05 ). HC gp - 39 was positively correlated with PLT ( r - 0. 310, P 〈 0.05 ). Conclusion NT- proBNP, HeS and HC gp -39 have an important role in Kawasaki's disease, NT - proBNP is associated with left ventricular systolic function in children, and HeS and HC gp -39 are related to platelet parameters in children.
