非瓣膜性心房颤动患者中性粒细胞/淋巴细胞比值与CHA_2DS_2-VASc评分的相关性分析 被引量:6

摘要 目的探讨非瓣膜性心房颤动患者中性粒细胞/淋巴细胞比值(NLR)与CHA_2DS_2-VASc评分的相关性。方法选择非瓣膜性心房颤动患者165例,按照纳入时NRL的中位数(2.54)分为低NLR组(84例)和高NLR组(81例)。比较两组一般临床资料并分析NLR与CHA_2DS_2-VASc评分的关系。结果高NRL组高血压病史比例、左前降支、血清Cr、TG、血清CRP、NEU、NLR均高于低NRL组,LVEF、Hb低于低NRL组,两组比较差异均有统计学意义(均P<0.05)。高NLR组CHA_2DS_2-VASc评分分级、CHA_2DS_2-VASc评分平均分值与低NRL组比较,差异均有统计学意义(均P<0.05)。Spearman秩相关分析显示,NLR与CHA_2DS_2-VASc、左心房内径(LAD)、CRP亦呈正相关(r=0.243、0.175、4.234,P<0.05),与左心室射血分数(LVEF)呈负相关(r=-0.196,P<0.05)。结论非瓣膜性心房颤动患者NLR与CHA_2DS_2-VASc评分存在相关性。 Objective To investigate the correlation between neutrophils to lymphocyte ratio(NLR) and CHA2DS-VASc score in patients with nonvalvular atrial fibrillation. Methods 165 patients with non-valvular atrial fibrillation were enrolled in our hospital from May 2014 to May 2017 and divided into low NLR group and high NLR group according to the median of NLR. Clinical data were compared between the two groups. The relationship between NLR and CHA2DS2-VASc score was analyzed. Results Hypertension rates, LAD, serum Cr, TG, CRP and NEU were significantly higher, LVEF and Hb were significantly lower in high NLR group than in low NLR group (all P〈0.05). CHA2DS2-VASc score grade(U=4.236, P〈0.05) and the average score of CHA2DS2-VASc(t=5.895, P〈0.05) were significantly higher in high NRL group than in low NRL group. Spearman rank correlation analysis showed that NLR was positively related with CHA2DS2-VASc score, LAD and CRP(r=0.243、0.175、4.234, P〈0.05) and negatively related with LVEF (r=-0.196, P〈0.05). Conclusion There is a certain correlation between NLR and CHA2DS2-VASc score in patients with nonvalvular atrial fibrillation.
