综合护理对老年股骨颈骨折患者术后疼痛及焦虑、抑郁的影响

Effects of comprehensive nursing on postoperative pain,anxiety,and depression in elderly patients with femoral neck fracture

目的:观察综合护理对老年股骨颈骨折患者术后疼痛及焦虑、抑郁的影响。方法:将2016年3月~2017年5月在我科接受全髋关节置换术的106例老年股骨颈骨折患者随机分为2组,其中对照组50例,采用常规护理;观察组56例,给予综合护理。采用视觉模拟评分法(Visual analogue scale,VAS)评估术后最早出现疼痛的时间、疼痛评分及术后12、48和72h的疼痛评分;汉密尔顿焦虑量表(Hamilton anxiety scale,HAMA)和抑郁量表(Hamilton depression scale,HAMD)比较患者术后4w焦虑、抑郁程度。结果:观察组术后出现疼痛的时间明显晚于对照组,疼痛评分低于对照组(P<0.01),术后12、48和72h的疼痛评分均明显低于对照组(P<0.05);术后4w焦虑、抑郁评分均明显低于对照组(P<0.05)。结论:综合护理可以明显缓解老年股骨颈骨折患者术后疼痛,改善焦虑、抑郁等不良心理状态,促进患者早日康复。

Objective：To evaluate the effects of comprehensive nursing on postoperative pain,anxiety,and depression in elderly patients with femoral neck fracture.Methods：One hundred and six elderlypatients with femoral neck fractures who underwent total hip arthroplasty from March 2016 to May 2017 in our hospitalwere randomly divided into two groups.Patients in thecontrol group（n=50）were given conventional nursing.And patients inthe observation group（n=56）were givencomprehensive nursing.The time of first pain,pain score,and pain scores at 12 h,48 hand 72 hpostoperationwere evaluated by visual analogue scale（VAS）.Anxiety anddepression were compared 4 weeks after surgery accordingto the Hamilton Anxiety Scale（HAMA）and the depression scale（HAMD）,respectively.Results：Compared with the control group,the occurrence time of pain was significantly later in the observation group.The pain score was significantly decreased in the observation groupthan that of the control group（P〈0.01）.The pain scores at 12 h,48 h,and 72 hwere significantly decreasedin the observation group than those of the control group（P〈0.05）.Moreover,the scores of anxiety and depression were significantly decreasedin the observation group than those of the control group（P〈0.05）.Conclusion：Comprehensive nursing can relieve the postoperative pain,improve the anxiety and depression,and promote the early recovery of the patients with femoral neck fracture.
