中药调脂方对血液透析患者脂质代谢紊乱的临床研究

Clinical study of Chinese herbal prescription on lipid metabolic disorders in hemodialysis patients

目的探究中药调脂方治疗血液透析患者脂质代谢紊乱的临床效果。方法选择该院2015年11月至2017年4月收治的70例血液透析患者作为研究对象,将其随机分成对照组和治疗组,每组35例。对照组采用辛伐他汀治疗,治疗组采用自拟调脂方联合辛伐他汀治疗,比较两组患者治疗前后的血脂水平、血清炎性因子水平、肾功能和肝功能水平、治疗的总有效率及不良反应发生率。结果治疗前,两组患者三酰甘油(TG)、总胆固醇(TC)、高密度脂蛋白胆固醇(HDL-C)、低密度脂蛋白胆固醇(LDL-C)等血脂指标,C反应蛋白(CRP)、白细胞介素6(IL-6)、肿瘤坏死因子α(TNF-α)等血清炎性因子,以及肾功能指标[尿素氮(BUN)、肌酐(Scr)]、肝功能指标[丙氨酸氨基转移酶(ALT)、天门冬氨酸氨基转移酶(AST)]比较,差异均无统计学意义(P>0.05);治疗后,治疗组患者的TG、TC、LDL-C、CRP、IL-6、TNF-α、BUN、Scr、ALT、AST等均明显低于对照组,治疗组患者的HDL-C水平明显高于对照组,治疗组患者治疗的总有效率明显高于对照组,不良反应发生率明显低于对照组,差异均有统计学意义(P<0.05)。结论对血液透析患者采用中药调脂方治疗,调脂效果显著,值得推广。

Objective To explore the clinical effect of traditional Chinese medicine on lipid metabolism disorders in hemodialysis patients.Methods 70 patients with hemodialysis received in the hospital from November 2015 to April 2017 were enrolled in the study.They were randomly divided into control group and treatment group with 35 cases in each group.The control group was treated with simvastatin and and the treatment group was treated with self-made fat recipe combined with simvastatin.The levels of serum lipid,serum inflammatory cytokines,renal function and liver function,the total effective rate of treatment and the incidence of adverse reactions were compared between the two groups before and after treatment.Results There were no statistical significances in serum lipid indexes such as triglyceride（TG）,total cholesterol（TC）,high density lipoprotein cholesterol（HDL-C）,low density lipoprotein cholesterol（LDL-C）,C reactive protein（CRP）,interleukin 6（IL-6）,tumor necrosis factorα（TNF-α）and other serum inflammatory factors,and renal function indexes,including urea nitrogen（BUN）,creatinine（Scr）,liver function indexes,including alanine aminotransferase（ALT）,aspartate aminotransferase（AST）between two groups before treatment（P〈0.05）;after treatment,the TG,TC,LDL-C,CRP,IL-6,TNF-α,BUN,Scr,ALT and AST levels of the patients in the treatment group were significantly lower than those in the control group;the HDL-C level of the patients in the treatment group was significantly higher than that in the control group;the total effective rate of the treatment group was significantly higher than that in the control group;the incidence of adverse reactions was significantly lower than that of the control group,and the differences were statistically significant（P〈0.05）.Conclusion The lipid-lowering effect of treatment of hemodialysis patients with traditional Chinese medicine lipid-lowering treatment is remarkable,and worthy of promotion.
