妇科急腹症的超声诊断及误诊原因

Ultrasonic diagnosis of gynecologic acute abdomen and the cause of misdiagnosis

目的:探讨各种类型妇科急腹症的超声诊断特点,以提高正确诊断率。方法:选择妇产科急腹症患者169例,回顾性分析其临床资料。结果:超声诊断符合率为94.88%。遗漏病例主要为异位妊娠、黄体破裂、盆腔及输卵管炎症。结论:对妇科急腹症,超声检查诊断有检查迅速、准确率高、没有痛苦且易于重复等优点,而经阴道及腹部联合超声检查具有更高的特异性及准确率。此外,在面对一些不典型症状及声像图时容易出现误诊,所以应仔细辨别声像特点的差异性。

Objective：To investigate the diagnostic characteristics of various types of gynecologic acute abdomen in order to improve the correct diagnosis rate.Methods：169 cases of acute abdomen in gynecology and obstetrics were selected.We retrospectively analyzed the clinical data.Results：The coincidence rate of ultrasonic diagnosis was 94.88%.Missed cases were mainly ectopic pregnancy,corpus luteum rupture,pelvic and oviduct inflammation.Conclusion：For gynecologic acute abdomen,ultrasonography was quick,accurate,painless and easy to repeat.Transvaginal and abdominal ultrasonography had higher specificity and accuracy.In addition,it was easy to misdiagnose in the face of some atypical symptoms and sonograms.So we should carefully distinguish the difference of the audio-visual characteristics.
