10+X针法超声引导下经直肠前列腺穿刺活检术86例分析

Analysis of 86 cases treated with 10+X needle-guided transrectal prostate biopsy

目的:探讨10+X针法超声引导下经直肠前列腺穿刺活检术对诊断前列腺癌的价值及安全性。方法:收治可疑前列腺癌患者行10+X针法经直肠超声引导下前列腺穿刺活检术86例,对术后病理结果及并发症做相关分析。结果:确诊前列腺癌患者27例(阳性率31.40%),其中PSA≤20 ng/m L、PSA 20~40 ng/m L、PSA≥40 ng/m L时,阳性率分别为10.53%、25.0%、48.57%;在PV≤50 m L、PV 50~70 m L、PV≥70 m L时,阳性率分别为31.71%、44.44%、11.11%。并发症包括血尿19例(22.09%),便中带血15例(17.44%),排尿困难3例(3.49%),急性尿潴留1例(1.16%),发热伴尿频尿急1例(1.16%)。结论:10+X针法超声引导下经直肠前列腺穿刺活检术能够较好地满足临床需求,可以作为初次前列腺穿刺活检的术式之一。

Objective：To explore the value and safety of transrectal prostate biopsy under the guidance of 10＋X needle-guided ultrasound in the diagnosis of prostate cancer.Methods：86 patients with suspicious prostate cancer underwent 10 ＋ X needle-guided transrectal prostate biopsy were selected.The postoperative pathological findings and complications were analyzed.Results：Twenty-seven patients with confirmed prostate cancer（the positive rate was 31.40%）.When PSA≤20 ng/m L,PSA 20~40 ng/m L,PSA≥40 ng/m L,the positive rates were 10.53%,25.0%,and 48.57% respectively.The positive rates were 31.71%,44.44%and 11.11% for PV≤50 m L,PV 50~70 m L and PV≥70 m L respectively.Complications included hematuria in 19 patients（22.09%）,fecal blood in 15 patients（17.44%）,dysuria in 3 patients（3.49%）,acute urinary retention in 1 patient（1.16%）,fever in 1 patient with urinary frequency urgency（1.16%）.Conclusion：10＋X needle ultrasound guided transrectal prostate biopsy can meet the clinical needs and can be used as one of the primary prostatic biopsy methods.
