精神分裂症患者自我怜悯与童年期创伤的相关性研究

Relationship between Self-Compassion and Childhood Trauma in Schizophrenia

目的探讨精神分裂症患者自我怜悯能力受损情况及其与童年期创伤经历之间的关系。方法选取精神分裂症患者200例(患者组),健康对照206例(正常对照组)。采用中文版自我怜悯量表(SCS-C)对两组的自我怜悯能力进行测定,采用童年期创伤问卷(CTQ)、阳性和阴性综合征量表(PANSS)、Beck抑郁量表(BDI)评估患者的童年期创伤水平、精神症状及抑郁症状。结果患者组SCS-C总分及各因子分均低于正常对照组(P<0.05);相关分析显示,CTQ中情感虐待因子分与PANSS阳性症状分呈正相关(P<0.01),SCS-C总分及各因子分均与BDI评分呈负相关(P<0.01),CTQ各因子分均与BDI评分呈正相关(P<0.01)。患者组CTQ情感虐待、情感忽视、躯体忽视因子分与SCS-C总分及各因子分均呈负相关(P<0.01),CTQ性虐待因子分与SCS-C总分及自我友善、普遍人性因子分呈负相关(P<0.01)。在控制BDI评分后的偏相关分析显示,CTQ情感忽视因子与SCS-C总分及自我友善因子分仍呈负相关(P<0.05)。结论精神分裂症患者存在自我怜悯能力受损,童年期创伤经历尤其是情感忽视可能会对患者的自我怜悯能力产生负面影响。

Objective To explore the deficits of self-compassion in patients with schizophrenia and its relationship with childhood trauma. Methods A total of 200 patients with schizophrenia and 206 normal controls were recruited. They were assessed with Self-compassion Scale（ SCS-C）,Childhood Trauma Questionnaire（ CTQ）,Positive and Negative Syndrome Scale（ PANSS） and Beck Depression Inventory（ BDI）. Results The total score and all the factor scores of SCS-C in patients group were significantly lower than those in control group（ P〈0. 05）. The emotion abuse score of CTQ was positively correlated with the positive syndrome score of PANSS（ P〈0. 01）. The total score of BDI was negatively correlated with the total and factor scores of SCS-C（ P〈0. 01） and positively correlated with the factor scores of CTQ（ P〈0. 01）. The scores of emotion abuse,emotion neglect and physical neglect of CTQ were negatively correlated with the total and factor scores of SCS-C（ P〈0. 01） while the sex abuse score of CTQ was negatively correlated with the total score and self-kindness and common humanity factor scores of SCS-C（ P〈0. 01）. When BDI score was controlled,the emotion neglect score was still negatively correlated with the total score and self-kindness factor score of SCS-C（ P〈0. 05）. Conclusion The self-compassion was impaired in patients with schizophrenia. Childhood trauma,especially emotion neglect,may have negative influence on patients＇ self-compassion.
