西洛他唑联合阿司匹林用于脑梗死介入治疗的临床观察

The Clinical Observation on Cilotazole Combined With Aspirin in Cerebral Infarction Interventional Therapy

目的通过观察经颅多普勒超声(TCD)监测到的微栓子信号数量,来研究西洛他唑联合阿司匹林用于脑梗死神经介入治疗中的有效性及安全性。方法将34例脑梗死患者随机分两组,分别应用西洛他唑联合阿司匹林肠溶片及氯吡格雷联合阿司匹林肠溶片治疗脑梗死支架植入血管成形术的患者,分别记录术后各阶段患者动脉大脑中动脉或大脑后动脉的微栓子数量,同时统计术后并发症情况。结果 34例患者均监测到微栓子信号,其中以神经介入治疗术后1~3天最多,7日后微栓子数量减少,两组患者减少比率差异无统计学意义,但术后并发症西洛他唑组较少,差异有统计学意义(P<0.05)。结论西洛他唑联合阿司匹林用于神经介入的治疗能有效预防血栓形成,效果不亚于氯吡格雷,且不良反应发生率较少。

Objective The efficacy and safety of cilostatazole combined with aspirin in the interventional treatment of cerebral infarction were studied by observing the number of microemboli signals detected by transcranial doppler ultrasound（TCD）. Methods 34 patients with cerebral infarction were randomly divided into two groups, respectively applied cilotazole combined aspirin enteric-coated tablets and clopidogrel in combination with aspirin enteric-coated tablets for angioplasty, the number of micro embolus in middle cerebral artery or posterior cerebral artery was recorded in different postoperative stages of patients, and statistics of postoperative complications. Results 34 patients were detected micro embolus signal, of which neurointervention for one to three days after surgery was the most, seven days after surgery was the least, the reduced ratio had no obvious difference, but there were fewer postoperative complications in cilotazole group, the difference was statistically significant（P〈0.05）. Conclusion The treatment of cilotazole combined with aspirin for neurointervention can effectively prevent thrombosis, which is no less effective than clopidogrel, and has fewer adverse reactions.
