全胸腔镜手术与传统开胸手术治疗食管癌的近期临床疗效比较

Comparison on short-term clinical effects of totally thoracoscopic surgery and traditional open-heart surgery in the treatment of esophageal cancer

目的比较全胸腔镜手术与传统开胸手术治疗食管癌的近期临床疗效。方法研究对象选取我院2013年6月~2016年6月收治的胸中段食管癌患者64例,其中,行全胸腔镜手术的患者32例为全胸腔镜组,其余32例行传统开胸手术的患者为对照组,比较两组患者各项手术观察指标。结果全胸腔镜组的手术时间(123.8±25.1)min、术中出血量(172.1±30.3)m L、胸引流管保留时间(3.1±1.1)d、住院时间(15.6±2.7)d,分别与对照组比较,组间比较差异存在显著性(P<0.05)。全胸腔镜组术后并发症发生率9.3%,显著低于对照组术后并发症发生率31.3%,组间比较差异存在显著性(P<0.05)。全胸腔镜组的疼痛程度较对照组显著减轻,组间比较差异存在显著性(P<0.05)。结论与传统开胸手术比较,全胸腔镜手术治疗食管癌具有出血少、创伤小、并发症少、疼痛轻等优点,值得在临床广泛推广和应用。

Objective To compare the short-term clinical effects of totally thoracoscopic surgery and traditional openheart sugery in the treatment of esophageal cancer. Methods 64 patients with middle thoracic esophageal cancer admitted in our hospital from June 2013 to June 2016 were selected. Among them, 32 patients underwent totally thoracoscopic surgery were identified as totally thoracoscopic group and the remaining 32 patients underwent traditional openheart surgery were identified as control group. We compared various surgical indicators of patients in the two groups.Results In the totally thoracoscopic group, the duration of operation was（123.8±25.1） min, the amount of blood loss during surgery was（172.1±30.3） m L, the retention time of chest drainage tube was（3.1±1.1） d and the duration of hospitalization was（15.6±2.7） d. Compared with the control group, these indicators showed significant difference（P〈0.05）. The incidence of postoperative complications in the totally thoracoscopic group was 9.3%, which was significantly lower than that in the control group（31.3%）（P〈0.05）. The pain severity of totally thoracoscopic group reduced significantly than the control group（P〈0.05）. Conclusion Compared with the traditional open-heart surgery, totally thoracoscopic surgery for esophageal cancer has advantages of less bleeding, less trauma, few complications and less pain, which is worthy of promoting and using widely in clinic.
