以目标血管对比剂峰值浓度和持续时间计算的低剂量对比剂在头颈部CTA中的应用 被引量:16

摘要 目的探讨在头颈部CTA中降低对比剂使用量的可行性方案。方法通过不同浓度对比剂对应的CT值,估算目标血管内所需对比剂浓度。根据既往学者研究结果,确定最佳头颈部CTA血管显示相对应的对比剂浓度。依据心输出量、CT机扫描时间、至目标血管循环的血容量计算对比剂使用总量。将本院68例头颈部CTA患者按随机量表分为两组,低对比剂组给予低对比剂量,对照组按常规剂量行CTA扫描。测量颈动脉分叉处CT值达到360HU为合格,达到460HU为最佳,分别进行统计学比较。结果头颈部CTA中,低剂量组对比剂剂量减少约50%。目标血管CT值合格率和良好率与对照组无统计学差异。结论在头颈部CTA检查中根据个体目标血管对比剂峰值浓度和持续时间计算的低对比剂使用量,可以在保证成像质量的前提下个体化的提供扫描方案,使对比剂的使用适量。 Objective To study the feasibility of the scheme of lowering usage of contrast agents in head and neck CTA.Methods Through the different concentration of contrast medium corresponding CT value,we estimated the required concentration of target intravascular contrast medium. According to the results from the previous researches,we determined the corresponding concentration of contrast agent that was the best for head and neck CTA. On the basis of cardiac output,CT scanning time,and the total blood volume to the target vascular circulation,we calculated the concentration of contrast medium. 68 patients,who underwent head and neck CTA in our hospital,were randomly divided into two groups: the low contrast group given low dose contrast and the control group given regular doses for CTA scanning. 360 HU of CT value measured at the carotid bifurcation was considered to be qualified and 460 HU,to be the best. And then,the comparison was statistically conducted respectively. Results As for the head and neck CTA,the contrast agent dose in low dose group was reduced by about 50%. The eligibility or good rate of CT value for target vascula had no statistical difference compared with that in the control group. Conclusion In the head and neck CTA inspection,the usage of low contrast agent dose according to contrast medium concentration from the individual target blood vessels and duration of peak value can ensure the quality of imaging of the individualized scanning solution,making the use of contrast medium is appropriate.
