糖化血红蛋白即时检验在妊娠糖尿病筛查中的应用价值

Application Value of Instant Test of Glycosylated Hemoglobin in Screening of Gestational Diabetes

目的研究糖化血红蛋白即时检验在妊娠糖尿病筛查中的应用价值。方法选择2015年10月—2016年10月在该院进行治疗的50例妊娠糖尿病患者为观察组,选择同年在该院检查的50名健康孕妇为对照组。对两组患者进行空腹血糖(FPC)、口服葡萄糖50 g筛选(GCT)和糖化血红蛋白(Hb Alc)即时检验,对比3种筛选方式在妊娠糖尿病筛选过程中的应用效果。结果观察组的空腹血糖、口服50 g葡萄糖和糖化血红蛋白即时检测比对照组高,数据差异有统计学意义(P<0.05);糖化血红蛋白在妊娠糖尿病筛选过程中的准确性、敏感性、阳性预测值、特异性、阴性预测值和Youden指数分别为90.2%、81.2%、89.9%、95.9%、91.4%、76.9%;糖化血红蛋白除了阴性预测值及敏感性比口服50 g葡萄糖筛选检测要低,其他指标均比空腹血糖检测及口服50 g葡萄糖筛选检测要高。结论糖化血红蛋白即时检验在妊娠糖尿病筛查过程中具有较高的应用价值,并且实用且便捷,不会受到其他因素的干扰,能够实现随时测定,值得临床应用推广。

Objective To research the application value of instant test of glycosylated hemoglobin in screening of gestational diabetes. Methods 50 cases of patients with gestational diabetes treated in our hospital from October 2015 to October 2016 were selected as the observation group, while 50 cases of healthy pregnant women examined in our hospital were selected as the control group, both groups used the FPC, 50 g GCT and Hb Alc instant test, and the application effect of the three methods was compared. Results The FPC, 50 g GCT and Hb Alc instant test values in the observation group were higher than those in the control group, and the differences were statistically significant(P<0.05), and the accuracy, sensitivity, positive prediction value, specificity, negative predication value and Youden index of the glycosylated hemoglobin in the screening course of diabetes were respectively 90.2%,81.2%,89.9%,95.9%,91.4%,76.9%, and the negative predication value and sensitivity of glycosylated hemoglobin were lower than those of 50 g GCT, and the other indicators were higher than those of FPC and 50 g GCT. Conclusion The application value of instant test of glycosylated hemoglobin in screening of gestational diabetes is higher, with simple operation, which can not be interfered by other factors, and it can realize the measurement at any moment, which is worth clinical promotion and application.
