浅析强化胰岛素治疗对脑出血伴糖尿病患者的预后影响

Analysis of Effect of Enhanced Insulin Therapy on the Prognosis of Patients with Cerebral Hemorrhage and Diabetes

目的探讨分析加强胰岛素对脑出血伴糖尿病患者的临床治疗效果和预后效果。方法根据2014年6月—2016年6月该院的脑出血伴糖尿病患者60例进行研究分析,患者分成了观察组和对照组,均有30例,对照组和观察组患者都接受了常规降糖和强化胰岛素治疗方式。对患者进行了血糖、神经功能缺损以及预后情况评估,对比分析两组的临床治疗效果。结果患者空腹血糖和餐后2 h血糖对比显示,观察组要比对照组优秀(P<0.05);对比两组神经功能缺损情况,观察组的评分要比对照组低(P<0.05);观察组患者的残疾率、死亡率分别是20.0%、10.0%,对照组的残疾率和死亡率是36.7%、20.0%,观察组患者的两项指标要比对照组低(P<0.05),观察组有70.0%患者恢复较好,对照组有43.3%患者恢复较好,观察组的恢复较好比例比对照组高(P<0.05)。结论脑出血伴发糖尿病患者接受强化胰岛素治疗能够对患者的预后起到较大的改善效果,患者的残疾率和死亡率均比较低,患者的神经功能也得到了改善,血糖水平正常,临床中可以广泛使用。

Objective To study and analyze the effect of enhanced insulin therapy on the prognosis of patients with cerebral hemorrhage and diabetes. Methods 60 cases of patients with cerebral hemorrhage and diabetes in our hospital from June2014 to June 2016 were selected and divided into two groups with 30 cases in each, both groups received the routine hypoglycemic therapy and enhanced insulin therapy, and the blood glucose, never function defect and prognosis of patients were evaluated, and the clinical treatment effect of the two groups was compared and analyzed. Results The comparison showed that the blood glucose and postprandial 2 h blood glucose in the observation group were better than those in the control group（P0.05）, and the nerve function defect in the observation group was lower than that in the control group（P0.05）,and the disability rate and death rate in the observation group were lower than those in the control group（20.0%,10.0% vs36.7%,20.0%）（P0.05）, and the recovery rate in the observation group was higher than that in the control group（70.0% vs43.3%）（P0.05）. Conclusion The enhanced insulin therapy in patients with cerebral hemorrhage and diabetes can greatly improve the prognosis with lower disability rate and death rate, and improve the nerve function and the blood glucose level is normal, and it can be widely applied in clinic.
