家庭医生团队签约服务对慢性病健康管理的效果评价

Evaluation of the Effect of Family Doctor Team Signing Service on the Health Management of Chronic Diseases

目的总结家庭医生团队签约服务对于慢性病健康管理的应用方法以及应用效果。方法回顾性分析2016年1月—2017年1月收治的100例慢性病患者资料,对其进行分组,研究组接受家庭医生团队签约服务,对照组接受常规服务,对比两组治疗效果。结果两组疾病知识知晓率、血压、血糖水平比较差异有统计学意义(P<0.05)。结论在为慢性病患者开展健康管理工作期间,家庭医生团队签约服务能够显著提升管理效果,改善患者的血压与血糖水平,应该给予大力的推广与应用。

Objective This paper tries to summarize the application methods and application effects of family doctor team signing services for the health management of chronic diseases. Methods A retrospective analysis of 100 patients with chronic disease data was conducted and grouped.from January 2016 to January 2017 The study group was contracted by a team of family doctors. The control group received routine services and the treatment effects of the two groups were compared. Results There was a statistically significant difference between the two groups in disease knowledge, blood pressure,and blood glucose levels（P〈0.05）. Conclusion While carrying out health management work for patients with chronic diseases,the signing service of the family doctor team can significantly improve the management results and improve the blood pressure and blood glucose levels of patients, and should be given great promotion and application.
