摘要
目的研究斑秃患者外周血T淋巴细胞亚群(CD3^+、CD4^+、CD8^+)数量和微量元素(Cu、Zn、Ca、Fe、Mg、Pb)浓度,及其与脱发面积严重程度的关系。方法选取我院门诊确诊的斑秃患者50例为观察组和30例健康体检者为对照组,采用免疫荧光法检测外周血T淋巴细胞亚群数量及采用原子吸收法检测微量元素的浓度,并与斑秃患者的脱发面积进行相关性分析。结果 (1)斑秃患者外周血CD3^+、CD4^+、CD8^+数量高于对照组(P<0.05)。(2)斑秃患者外周血Zn、Ca、Fe、Mg浓度明显低于对照组(P<0.01);斑秃患者外周血Cu、Pb浓度高于对照组(P<0.05)。(3)斑秃患者脱发面积与外周血CD3^+、CD4^+、CD8^+数量间呈直线正相关关系(P<0.01);斑秃患者脱发面积与外周血Mg、Pb浓度间呈直线正相关关系(P<0.05);斑秃患者脱发面积与外周血Cu、Zn、Ca、Fe浓度间无线性关系(P>0.05)。结论 T淋巴细胞亚群(CD3^+、CD4^+、CD8^+)及微量元素(Cu、Zn、Ca、Fe、Mg、Pb)可能参与了斑秃的发病,而T淋巴细胞亚群(CD3^+、CD4^+、CD8^+)与微量元素(Mg、Pb)与脱发面积严重程度有一定的关系。
Objective To conduct a study on the number of T-lymphocyte subpopulation(CD3+,CD4+,CD8+) and the concentration of trace elements(Cu,Zn,Ca,Fe,Mg,Pb) in peripheral blood for patients with alopecia areata as well as their relations with the severity of alopecia area. Methods 50 patients diagnosed as alopecia areata in the outpatient department of Dermatology Hospital of Jiangxi Province have been selected as the case group and 30 healthy people have been selected as the control group. Immunofluorescence is used to detect the number of T-lymphocyte subpopulation in peripheral blood;atomic absorption spectroscopy is adopted to detect the concentration of trace elements in peripheral blood. Based on this,a correlation analysis has been made on their connections with the area of hair loss for patients with alopecia areata. Results(1)The number of CD3+,CD4+and CD8+in peripheral blood for patients with alopecia areata is higher than that for the control group(P〈0.05). (2)The concentration of Zn,Ca,Fe,Mg in peripheral blood for patients with alopecia areata is significantly lower than that for the control group(P〈0.01); the concentration of Cu and Pb in peripheral blood for patients with alopecia areata is higher than that for the control group(P〈0.05). (3)It is shown that there is a linear positive correlation between the area of hair loss for patients with alopecia areata and the number of CD3+,CD4+and CD8+in peripheral blood(P〈0.01);there is a linear positive correlation between the area of hair loss for patients with alopecia areata and the concentration of Mg and Pb in peripheral blood(P〈0.05);there is no linear correlation between the area of hair loss for patients with alopecia areata and the concentration of Cu,Zn,Ca,Fe in peripheral blood(P〈0.05). Conclusion T-lymphocyte subpopulation(CD3+,CD4+,CD8+) and trace elements(Cu,Zn,Ca,Fe,Mg,Pb) may contribute to the pathogenesis of alopecia areata. There is a connection between T-lymphocyte subpopulation(CD3+,CD4+,CD8+) and trace elements(Mg and Pb)and the severity of hair loss.
