活动性肺结核患者外周血CD4^+T细胞中TLR4与血清IFN-γ、IL-4的关系 被引量:15

摘要 目的探究活动性肺结核(PTB)患者外周血CD4^+T细胞中Toll样受体4(TLR4)与γ干扰素(IFN-γ)、白细胞介素4(IL-4)的关系。方法选取我院收治的95例活动性PTB患者为研究对象,将本组患者根据疾病严重程度分为轻度组、中度组和重度组,同期选取30例体检健康者为对照组。分别检测各组患者外周血CD4^+T细胞中TLR4表达情况和血清IFN-γ、IL-4水平。结果 PTB轻度、中度、重度组CD4^+T细胞TLR4 mRNA表达量明显高于对照组,重度组也明显高于轻度和重度组(P<0.05);轻中度和重度组血清IFN-γ水平均显著低于对照组,其中重度组明显低于其他两组(P<0.05);轻中度和重度组血清IL-4水平均显著高于对照组,其中重度组明显高于其他两组(P<0.05);Pearson相关性分析,肺结核患者CD4^+T细胞TLR4mRNA表达情况与血清IFN-γ水平呈负相关(P<0.05);与IL-4水平呈正相关(P<0.05)。结论 PTB患者外周血CD4^+T细胞TLR4表达显著升高,且与血清IFN-γ、IL-4水平有显著相关性,推测外周血CD4^+T细胞TLR4表达可能参与PTB的发生发展。 Objective To investigate the relationship of TLR4 with IFN-γ and IL-4 in peripheral blood CD4^+T cells in patients with active pulmonary tuberculosis( PTB). Methods 95 active PTB patients in our hospital were selected as the research objects,and they were divided into the mild group,the moderate group and the severe group according to the severity of the disease. And 30 healthy people were selected as the control group at the same time. The expression of TLR4 in peripheral blood CD4^+T cells and serum levels of IFN-γ and IL-4 were detected respectively. Results The expressions of TLR4 mRNA in CD4^+T cells were significantly higher in the mild,moderate and severe PTB groups than in the control group,and they were also significantly higher in the severe group than in the mild and severe groups( P〈0. 05). The level of serum IFN-γ in the mild,moderate and severe groups was significantly lower than that in the control group,and it was lower in the severe group than in the other two groups( P〈0. 05). The level of serum IL-4 in the mild,moderate and severe groups was significantly higher than that in the control group,and it was obviously higher in the severe group than in the other two groups( P〈0. 05). Pearson correlation analysis showed that the expression of TLR4 mRNA in CD4^+T cells was negatively correlated with serum IFN-γ( P〈0. 05),and positively correlated with IL-4( P〈0. 05). Conclusion The expression of TLR4 in peripheral blood CD4^+T cells of patients with PTB is significantly increased,and it is significantly correlated with serum levels of IFN-γ and IL-4,suggesting that the expression of TLR4 in peripheral blood CD4^+T cells may be involved in the development of PTB.
