痛风通络剂治疗湿热蕴结型急性痛风性关节炎临床观察

Clinical observation of Tongfeng tongluo preparation on acute gouty arthritis of heat-damp stagnating syndrome

目的:观察痛风通络剂联合西药治疗急性痛风性关节炎的临床疗效及对炎症指标的影响。方法:将70例急性痛风性关节炎患者随机分为治疗组和对照组各35例。对照组予双氯芬酸钠双释放肠溶胶囊口服,每次75mg,每日1次。治疗组在对照组基础上给予中药痛风通络剂每日1剂口服。两组疗程均为10天。观察两组治疗前后的关节疼痛、红肿症状评分及血沉、C反应蛋白变化。结果:治疗组总有效率94.29%(95%CI=80.58%~99.26%),对照组为85.71%(95%CI=79.79%~91.63%),两组综合疗效比较(u=0.7985,P=0.4245),差异未达到显著性意义;两组患者治疗后症状积分、C反应蛋白、血沉较治疗前降低,且治疗组优于对照组(P<0.01)。结论:痛风通络剂结合西药治疗急性痛风患者的疗效可能优于单纯西药,其收益为OR=0.36(95%CI=0.07~2.02),NNT=12(95%CI=4.42~29.91),有利于治疗组;明显改善患者关节红肿热痛症状,安全性好。

Objective：To observe the clinical effect of Tongfeng tongluo preparation combined with western medicine on acute gouty arthritis and the influence on inflammation indexes.Methods：70 cases of patients with acute gouty arthritis were randomly divided into treatment group and control group,35 cases in each group.The control group was treated with oral diclofenac sodium dual release eneric-coatde capsules,75 mg each time,once daily; while the treatment group was with oral Tongfeng tongluo preparation of traditional Chinese drugs on the basis of control group,1 dose per day.For both groups,the course of treatment was 10 days.Before and after treatment,the symptom scores of pain and redness-swelling of joints and the changes of erythrocyte sedimentation rate and Creactive protein were observed in two groups.Results：The total effective rate was 94.29%（ 95% CI = 80.58%-99.26%） in the treatment group and 85.71%（ 95% CI = 79.79%-91.63%） in the control group respectively.Compared the comprehensive effects of two groups（ u = 0.7985,P = 0.4245）,the difference did not reach significant significance.In two groups of patients,the symptom score,C-reactive protein and erythrocyte sedimentation rate after treatment were lower than those before treatment,and the treatment group was better than the control group（ P〈0.01）.Conclusion：The clinical effect of Tongfeng tongluo preparation with combined western medicine on acute gouty arthritis may be better than that of simple western medicine,and its benefit is OR = 0.36（ 95% CI = 0.07 - 2.02）,NNT = 12（ 95% CI = 4.42 - 29.91）,which is advantageous to improve significantly the symptoms of redness-swelling and thermal pain of patients＇ joints in treatment group,and with good safety.
