基于数据挖掘的灸法治疗遗尿症的现代文献选穴规律研究

Study of Acupoint Selection Rules in Modern Literatures of Moxibustion in Treatment of Enuresis Based on Datamining

目的运用数据挖掘技术分析灸法治疗遗尿症的取穴规律,为临床治疗提供参考。方法计算机检索中国知识资源总库(CNKI)、中国学术期刊数据库(万方数据)、中文科技期刊数据库(重庆维普)、中国生物医学文献服务系统(Sino Med)2007年1月1日至2017年1月1日灸法治疗遗尿症的临床研究文献,建立相应数据库,运用数据挖掘技术中的关联规则分析,分析灸法治疗遗尿症的临床选穴规律。结果共纳入文献63篇,共涉及灸法10种,以温和灸为主(24篇),共涉及腧穴44个,取穴总频次为300次,以任脉、足太阳膀胱经、足太阴脾经为主;耳穴18个,取穴总频次54次。腧穴主要取穴部位为腹部穴位,达103频次(38.4%)。关联规则分析显示支持度最高的是膀胱俞、百会、肾俞、中极、关元、三阴交;耳穴支持度最高的是膀胱,肾,皮质下。结论灸法治疗遗尿症多选用温和灸,遗尿症属于"前阴病",阴病要治阴,经脉穴位上多选取阴经,偏重特定穴,尤其重视交会穴的运用,结合辨证取穴,同时也注重耳穴的应用。

Objective To analyze the acupoint selection rules of moxibustion in treatment of enuresis based on datamining, and to provide references for clinical practice. Method Clinical research papers on moxibustion in treatment of enuresis were retrieved via computer from the China National Knowledge Infrastructure（CNKI）, Wanfant Database, Chongqing Vip Database and Sino Med in the range of January 1, 2007 and January 1, 2017. The literatures were collected to build up a database and association rules analysis of datamining was used to analyze the acupoint selectin rules of moxibustion in treatment of enuresis. Result A total of 63 articles were included, involving 10 types of moxibustion, majorly mild moxibustion（24 articles）, and 44 acupoints with a total frequency of 300 times, majorly points from the Conception Vessel, Bladder Meridian of Foot Taiyang and Spleen Meridian of Foot Taiyin; eighteen auricular points were involved with a total frequency of 54 times. The points located on abdomen were majorly selected and the frequency reached 103 times（38.4%）. The association rules analysis showed that Pangguangshu（BL28）, Baihui（GV20）, Shenshu（BL23）, Zhongji（CV3）, Guanyuan（CV4） and Sanyinjiao（SP6） had the highest support rate; among the auricular points, Pangguang（CO9）, Kidney（CO（10）） and Subcortex（AT4） had the highest support rate. ConclusionIn treatment of enuresis with moxibustion, mild moxibustion is often used. Enuresis is an external genitalia disease and should be treated by targeting the yin aspect. Therefore, acupoints from the yin meridians are mostly used. Specific acupoints are highlighted, especially the crossing and confluent points based on syndrome differentiation. Meanwhile,auricular points are also emphasized.
