香砂八珍汤联合肠内营养对消化道肿瘤术后患者免疫功能的影响

Effect of Xiangsha Bazhen Decoction combined with enteral nutrition on immune function of patients after digestive tract tumor operation

目的探讨香砂八珍汤联合肠内营养对消化道肿瘤术后患者免疫功能的影响。方法将100例消化道肿瘤术后患者随机分为2组,对照组50例给予肠内营养单用治疗,观察组50例在此基础上加用香砂八珍汤治疗,观察2组治疗前后中医证候积分、SF-36健康调查量表评分及T淋巴细胞亚群、自然杀伤细胞、免疫球蛋白、白蛋白(ALB)、前白蛋白(PA)、血红蛋白(Hb)、转铁蛋白(TRF)和缩胆囊素水平变化情况,统计2组临床疗效。结果 2组治疗后气短神疲、面色萎黄、乏力、头晕目眩及脉虚评分和CD8^+水平均显著降低(P均<0.05),躯干功能、角色功能、情感职能、社会功能、身体疼痛、生命活力、精神健康及总体健康评分、CD4^+、CD4^+/CD8^+、自然杀伤细胞、IgA、IgG、IgM、ALB、PA、Hb、TRF及缩胆囊素水平均显著提高(P均<0.05),且观察组治疗后以上指标改善情况均显著优于对照组(P均<0.05);观察组治疗总有效率明显高于对照组(P<0.05)。结论香砂八珍汤联合肠内营养用于消化道肿瘤术后患者可有效缓解相关症状体征,改善营养状态,提高机体免疫功能,并有助于刺激缩胆囊素分泌。

Objective It is to investigate the influence of Xiangsha Bazhen Decoction combined with enteral nutrition on the immune function of patients after digestive tract tumor operation. Methods One hundred patients with digestive tract tumor after operation were randomly divided into 2 groups. Fifty cases in the control group were treated with enteral nutrition alone,and fifty cases in the observation group were treated with Xiangsha Bazhen Decoction combined with enteral nutrition. The scores of TCM syndrome and SF-36 health survey,and the levels of T lymphocyte subsets,natural killer cells,immunoglobulin,albumin（ Alb）,prealbumin（ PA）,hemoglobin（ Hb）,transferrin（ TRF） and cholecystokinin level were observed before and after treatment. The clinical curative effects of the 2 groups were analyzed. Results After treatment,the scores of breath shortness and fatigue,sallow complexion,weakness,dizziness,pulse deficiency and the level of CD8~＋were significantly shorter in the 2 groups（ all P〈0. 05）,the scores of trunk function,role function,emotional function,social function,body pain,life vitality,mental health and overall health,and the levels of CD4~＋,CD4~＋/CD8~＋,natural killer cells,IgA,IgG,IgM,ALB,PA,Hb,TRF,and cholecystokinin were significantly increased（ all P〈0. 05）,and the improvement of above indexes in the observation group were significantly better than those in the control group（ P〈0. 05）. Conclusion Xiangsha Bazhen Decoction combined with enteral nutrition can effectively relieve symptoms and signs after digestive tract tumor surgery,and improve the quality of daily life and nutritional status,increase the immune function of the body,it also helps to stimulate the secretion of cholecystokinin.
