超声造影联合超声BI-RADS分级在临床疑似乳腺癌病变中的诊断价值

Value of Contrast-enhanced Ultrasound Combined with BI-RADS in the Diagnosis of Suspected Breast Cancer

目的探讨超声造影联合超声BI-RADS分级在临床疑似乳腺癌病变中的诊断价值。方法入选84例超声BI-RADS分级4~5级、疑似乳腺癌病变者,使用西门子S2000彩色多普勒超声诊断仪对其进行超声造影检查,以病理学结果为金标准,比较超声造影、BI-RADS分级以及二者联合检测对乳腺癌的诊断价值。结果本组84例经病理学诊断为良性61例,恶性23例。超声造影与BI-RADS分级联合检查法对乳腺癌的灵敏度、特异度、阳性预测值及阴性预测值分别为91.3%、95.5%、87.5%、96.7%,均高于单独检查法(P<0.05)。结论对于临床疑似乳腺癌病变者,采用超声BI-RADS分级联合超声造影诊断的可靠性高,可减少漏误诊。

bjective To investigate the value of contrast-enhanced ultrasound and BI-RADS in the diagnosis of suspected breast cancer. Methods 84 patients with 4~5 grade of BI-RADS and suspected breast cancer were enrolled in this study,and were conducted contrast enhanced ultrasound by SIEMENS S2000 color Doppler ultrasonic diagnostic apparatus,and pathological results was ptu as gold standard to compare the diagnostic value of contrast-enhanced ultrasonography,BI-RADS and 2 joint detection in breast cancer. Results Among 84 cases,61 cases were benign,accounting for 72.6%,and 23 cases were malignant,accounting for 27.4%.The sensitivity,specificity,positive predictive value and negative predictive value of contrast-enhanced ultrasound combined with BI-RADS were 91.3%,95.5%,87.5%,and respectively,which were significantly higher than those of the single test （ P 〈0.05）. Conclusion For patients with clinically suspected breast lesions,the use of ultrasound BI-RADS classification combined with contrast-enhanced ultrasound diagnosis has high reliability,and can reduce misdiagnosis and missed diagnosis.
