矽肺合并呼吸衰竭患者无创正压机械通气治疗效果观察

摘要 目的探究无创正压机械通气治疗矽肺合并呼吸衰竭的临床疗效。方法随机选择2014年10月~2017年10月间接受矽肺合并呼吸衰竭诊治的患者60例,患者入院后给予其无创正压机械通气治疗,观察和分析治疗效果。结果患者持续治疗时间为7~11 d,每日治疗时间为2~8 h,经过治疗后所有患者呼吸困难症状全部消失。TCO_2、PaCO_2均较应用呼吸机治疗前有明显下降,差异具有统计学意义(P<0.05),SaO_2、PaO_2均较治疗前有明显升高,差异具有统计学意义(P<0.05),治疗前后HCO_3^-变化不明显(P>0.05),p H值异常消失,与治疗前对比,差异具有统计学意义(P<0.05)。结论对矽肺合并呼吸衰竭患者应用无创正压机械通气治疗,可显著改善患者肺通气功能。 Objective To investigate the clinical efficacy of non-invasive positive pressure mechanical ventilation in the treatment of silicosis complicated with respiratory failure. Methods Sixty patients were randomly selected from October 2014 to October 2017 in our hospital for treatment of silicosis and respiratory failure. Patients were admitted to hospital for noninvasive positive pressure mechanical ventilation. The therapeutic effect was observed and analyzed. Results Patients continued treatment for 7 to 11 days, daily treatment time is 2 to 8 hours, after treatment, all patients with dyspnea symptoms disappeared. TCO2 and Pa CO2 were significantly lower than those before treatment(P 〈 0.05), Sa O2 and Pa O2 were significantly higher than before treatment, the difference was statistically significant(P 〈 0.05), changes in treatment There was no significant difference in HCO3-level before and after treatment(P 〉0.05), and the abnormality of p H value disappeared. There was a significant difference between before and after treatment(P 〈 0.05). Conclusion The application of noninvasive positive pressure mechanical ventilation in patients with silicosis and respiratory failure can improve pulmonary ventilation.
