金水宝胶囊联合AVAPS通气技术治疗稳定期慢性阻塞性肺疾病疗效及对免疫功能的影响

Effect of Jin Shui Bao capsule combined with AVAPS ventilation in the treatment of stable chronic obstructive pulmonary disease

目的探讨金水宝胶囊联合平均容积保证压力支持(AVAPS)通气技术治疗稳定期慢性阻塞性肺疾病的疗效及对患者免疫功能、肺功能的影响。方法将106例稳定期慢性阻塞性肺疾病患者随机分为中药组和对照组各53例。对照组给予鼻导管低流量吸氧、抗炎平喘、AVAPS无创通气等基础治疗;中药组除给予上述治疗之外,予以金水宝胶囊3粒/次口服,3次/d,疗程8周。治疗前后对患者呼吸困难、咳嗽、咳痰和双肺喘鸣音情况进行评分,检测血清IgM、IgG、IgA、CD4^+、CD8^+水平,测定第1秒用力呼气容积(FEV_1)、FEV_1%和第1秒用力呼气容积与用力肺活量百分比(FEV_1/FVC)。结果治疗8周后,中药组和对照组总有效率比较差异有统计学意义(P<0.05);2组呼吸困难、咳嗽、咳痰和双肺喘鸣音评分均较治疗前明显下降(P均<0.05),但中药组与对照组比较下降更明显(P均<0.05);治疗后2组FEV_1、FEV_1%和FEV_1/FVC均较治疗前明显升高(P均<0.05),中药组升高明显优于对照组(P均<0.05);2组血清IgM、IgG、IgA、CD4^+、CD4^+/CD8^+水平均较治疗前明显上升(P均<0.05),CD8^+水平较治疗前明显下降(P均<0.05),中药组改善情况明显优于对照组(P均<0.05)。结论金水宝胶囊联合AVAPS通气技术治疗稳定期慢性阻塞性肺疾病疗效确切,可缓解临床症状,改善肺功能,提高机体免疫力,值得临床推广应用。

Objective It is to investigate the effect of Jin Shui Bao capsule combined with AVAPS in the treatment of stable chronic obstructive pulmonary disease （COPD） and the influence on immune function and pulmonary function. Methods 106 patients with stable COPD were randomly divided into two groups, 53 cases in each group. The control group was given oxygen therapy, moxifloxacin anti infection, AVAPS, The observation group was given Jin Shui Bao capsule orally （3 pills/time, 3 times/d） based on the therapy in the control group. Both groups were treated for 8 weeks.Before and after treatment, the scores of dyspnea, cough, sputum, and double lung wheezing were evaluated, the levels of serum IgM, IgG, IgA, CD4^＋, and CD8^＋ were measured, FEV 1, FEV 1% and FEV 1/FVC was detected in both groups. Results After treatment for 8 weeks, the difference was statistically significant in the total effective rate between the two groups （ P 〈0.05）; the scores of dyspnea, cough, expectoration and double lung wheezing and the level of CD8^＋ were decreased significantly while FEV 1, FEV 1% and FEV 1/FVC and the level of serum IgM, IgG, IgA, CD4^＋, CD4^＋ and /CD8^＋ were increased in both groups （ P 〈0.05）, and the increase, decrease or improvements of these indexes were more significant in observation group than those in control group （ P 〈0.05）. Conclusion Jin Shui Bao capsule combined with AVAPS ventilation technology that can relieve clinical symptoms, improve lung function, and improve immunity has a definite curative effect on stable COPD, it is worthy of clinical application.
