摘要
目的研究彩色多普勒超声监测脑血流频谱变化在预测新生儿窒息及颅脑损伤中的价值。方法选择100例窒息新生儿,于出生后24 h、72 h进行颅脑超声检查,并测得每位新生儿大脑前动脉血流参数,并与同期收治的50例正常新生儿进行对比分析。结果观察组24 h后收缩期峰值血流速度(Vs)为(29.57±6.08)cm/s,舒张期峰值血流速度(Vd)为(11.55±2.21)cm/s,阻力指数(RI)为(0.85±0.13),搏动指数(PI)为(1.27±0.11),各项参数与对照组差异均有统计学意义(P<0.05)。结论彩色多普勒超声监测新生儿窒息时脑血流出现异常变化,表现为Vs、Vd降低,RI、PI升高,其脑动脉血流参数变化具有一定规律性,具有较高临床预测价值和实用价值。
Objective To study the value of color Doppler ultrasonography in monitoring the changes of cerebral blood flow spectrum in predicting neonatal asphyxia and traumatic brain injury. Methods 100 cases of neonatal asphyxia were measured by ultrasound for parameters of each newborn cerebral artery blood flow at 24 h, 72 h after birth as observation group, and 50 normal neonates were treated with the same period as the control group. Results After 24 h, the Vs, Vd, RI, PI of the observation group were (29.57 ±6.08) cm/s, (11.55 ± 2.21 ) cm/s, (0.85± 0.13 ), (1.27± 0.11 ) respectively. The differences of parameters in the observation group and the control group were statistically significant ( P 〈 0.05 ). Conclusion The results with certain regularity of color Doppler ultrasound monitoring in neonatal asphyxia cerebral blood flow showed abnormal changes including a decrease in Vs and Vd but an increase in RI and PI, which has high clinical predictive value and practical value.
